What Is Scarless (Closed) Rhinoplasty?
Scarless rhinoplasty, also known as closed nose job or more accurately “endonasal rhinoplasty,” is a sophisticated surgical technique that reshapes the nose through hidden incisions inside the nostrils, leaving no visible external scar on the columella.
Over the past 22 years at The Naderi Center, Dr. Shervin Naderi has performed thousands of both open and closed rhinoplasties. He spent decades of training and education achieving the prestigious distinction of becoming a double board-certified facial plastic surgeon. He went on to elevate his expertise and specialization by becoming an “ultra specialist” focusing exclusively on, and perfecting just one surgery – rhinoplasty. This is the only surgery Dr. Naderi performs, and the title of “Nose-man” is one no other surgeon in the U.S. can lay claims to. Patients from across the United States seek out his expertise for this highly specialized surgery – often called the most complex surgery of all.
The Evolution and Revival of Closed Rhinoplasty
Closed rhinoplasty is an older technique no longer taught at most U.S. Residency or Fellowship programs. It’s an art that has become lost. Closed rhinoplasty was very popular 50 years ago when plastic surgeons performed a very quick, abbreviated, and destructive nose job involving major removal of bone and cartilage resulting in eventual collapse and pinching and the telltale signs of a “bad nose job.” With the increased popularity of open rhinoplasty, plastic surgeons started utilizing more cartilage grafts to help support the areas they had resected and destroyed in the hopes of achieving better long-term results.
Unfortunately, many plastic surgeons perform today’s surgery the same way they trained and learned many years ago. A combination of lack of interest in continuing education as well as the “surgeon’s ego” often hinders progress and improvement. However, a true specialist analyzes his or her work constantly and evolves continuously to perfect the surgery and the results.
This is how Dr. Naderi is different than most of the rest. His technique today is dramatically different than his technique a decade or two decades ago. Dr. Naderi is one of a very small handful of facial plastic surgeons in the United States who combines Structural rhinoplasty with Preservation rhinoplasty using power instrumentation for precise maneuvers resulting in the least trauma and the best results. You will have to travel across many States or even countries to find someone with Dr. Naderi’s diverse skillset in both open and closed rhinoplasty.
Who Is a Good Candidate for Closed Scarless Rhinoplasty?
Ideal candidates for endonasal rhinoplasty include patients seeking subtle changes to the nose tip, bridge, or profile, with no need for major structural changes. The approach works best for patients who do not require extensive cartilage grafting or tip reshaping or need for addition of grafts and support. While camouflage grafts can easily be added during a closed nose job, structural grafts are best placed with the open (external) rhinoplasty.
What issues can closed rhinoplasty address?
- Minor dorsal hump reduction
- Narrowing of the nasal bones or bridge
- Refinement of the nasal tip (when changes are modest)
- Minor asymmetries especially after previous rhinoplasty
Patients who require complex revisions, major tip reshaping or repositioning, or have significant nasal trauma history may be better suited to an open rhinoplasty.

“I recently underwent rhinoplasty with Dr. Naderi, and the experience has been transformative. I consulted with several surgeons before, but the Naderi Center stood out for its exceptional reputation and cutting-edge technology. The computer-generated images they provided were a game-changer, allowing me to visualize and feel confident about the results before even starting. Dr. Naderi’s detailed walkthrough of his vision for me was both reassuring and impressive. I appreciated him setting realistic expectations as well. The surgery went smoothly, and the recovery process was well-supported by Dr. Naderi and his attentive staff. The results have exceeded my expectations, enhancing my confidence and self-esteem. Differences Between Open and Closed Rhinoplasty
On the surface it may appear that the primary difference lies in the location of the incisions. Closed rhinoplasty involves most of incisions placed inside the nostrils. Open rhinoplasty requires a very small additional incision across the columella (the strip of skin between the nostrils), which allows the surgeon to lift the nasal skin and gain full visibility of the underlying structure and its inherent asymmetries.
Advantages of Closed Rhinoplasty
- No external scar
- Faster healing and less swelling (not always a fair assessment because often with closed surgery less is being done, reflecting the quicker healing rather than a true difference between open vs. closed)
- Shorter operative time (again often because less is being done but in fact closed rhinoplasty can take longer due to the more limited visibility through the small nostrils)
- Quicker return to daily activities if the tip is unaltered.
Advantages of Open Rhinoplasty
- Full visualization of nasal anatomy
- Better suited for complex reconstructions or revisions
- Easier access for cartilage graft placement and structural support
Scar Management in Closed Nose Job
The very tiny “controversial” scar in open rhinoplasty is almost never a real issue because Dr. Naderi uses meticulous techniques to ensure the incision heals beautifully. The scar is usually placed in the narrowest portion of the columella and typically fades over time to become virtually undetectable. Scarless closed rhinoplasty eliminates this concern altogether. But even in closed rhinoplasty, if “alar base reduction” is needed then external incisions will be unavoidable. Here again, Dr. Naderi has a unique approach to alar base reduction (nostril narrowing), placing the external incisions in a unique hidden location that is different than most other plastic surgeons’ approach.

Cost of Closed Rhinoplasty
At The Naderi Center, our pricing is very open and transparent. All of our price ranges are listed on our website. Pricing can somewhat vary based on individual needs and complexity. Closed rhinoplasty can be a more technically complex surgery and incur higher cost but often that extra cost is offset by the fact Dr. Naderi offers closed nose jobs to those requiring more finesse, less involved work. Those requiring more work and longer surgeries are often advised toward open approaches.
Moreover, as a master surgeon, Dr. Naderi is decisive during surgery and does not aimlessly waste time during surgery. His technique looks beautiful like a choreographed dancer. This results in shorter operative times, less swelling, less scar tissue, quicker healing and lower hospital and anesthesia fees. Dr. Naderi’s team provides comprehensive price quotes after your consultation. For those seeking payment options, our financing plans can make surgery more accessible.
Previous Septoplasty or Functional Surgery
Patients who have undergone prior nasal procedures, such as rhinoplasty or even isolated septoplasty, often face higher costs for cosmetic rhinoplasty. Any previous surgery creates more challenges due to entering a nose with scar tissue and lack of native cartilages (removed by the previous surgeon).
Patients who have had previous septoplasty face a situation with reduced amount of usable septal cartilage for structural graft support making the use of rib cartilage a real possibility. (As a side note, you should be aware that when Dr. Naderi performs septoplasty, he carefully preserves cartilage and laminates remaining cartilages to strengthen the nasal framework and help prevent future deviation. This is his unique technique which he calls “structural septoplasty.”)
Choosing the Right Expert for Your Scarless Nose Job
If you are considering scarless (closed) rhinoplasty, trust your face to a true rhinoplasty expert. Dr. Naderi will carefully evaluate your anatomy and goals to determine if the closed approach is right for you. His honest, meticulous, and artistic technique is what sets The Naderi Center apart.

Avoiding Common Mistakes When Selecting a Surgeon
It would be a mistake to choose a surgeon who advertises and dogmatically offers 100% closed rhinoplasty all the time, or vice versa, only has trained in open rhinoplasty. A true rhinoplasty expert needs to be comfortable and familiar with both open and closed techniques as well as be capable of handling both cosmetic and breathing functional surgery. It would also be a mistake for a patient to choose and insist on the approach due to their online research or advice from friends and family.
It is wise and advisable to be seen for consultation by a true rhinoplasty specialist and seeing which approach they recommend for your anatomy and your goals. Too many plastic surgeons hype up and advertise the closed approach as “atraumatic” and claim it is a superior technique. This is simply not true. Every surgery involves trauma and every surgery involves incisions and scars. Closed technique simply skips one very small incision at the bottom of the nose. Closed rhinoplasty has its benefits as well as its limitations. Do not fall prey to the online marketing and hype. You owe it to yourself to see Dr. Naderi, a true skilled expert, who will discuss the pros and cons of both open and closed rhinoplasty with you and help you choose the best surgery for your nose.

FAQs about Closed Rhinoplasty
Can a scarless rhinoplasty surgery fix breathing issues too?
In many cases, it helps, especially if the breathing issue is small or linked to internal anatomy. Difficult functional problems need an open method for clear sight and repair.
What if I’ve already had a rhinoplasty procedure before?
Previous nasal surgery changes the internal structure and reduces the amount of cartilage. For some revisions, a closed approach works. But most revisions need an open approach, as it addresses scars, collapse, or asymmetry better.
Does a scarless nose job cost less than an open one?
The method of scarless rhinoplasty is often more difficult to do; it needs a surgeon with advanced skill. The final cost shows how complicated the case is and the surgeon's ability, not just where the incision is.
