Is Scarless Rhinoplasty Really Scarless? Where Every Nose Job Hides Its Incisions

Few phrases in cosmetic surgery sound as appealing as “scarless rhinoplasty.” It promises a new nose with no trace of how you got it. Patients from McLean, Great Falls, Bethesda and Northwest Washington often arrive at a consultation with that word already in mind, and it is a fair question to ask what it really means.

Here’s your guide to where the incisions go in every kind of nose job, which ones can be seen, and why the answer matters less than most people expect.

The short answer: a scarless rhinoplasty still uses incisions, but every one of them sits inside the nostrils, so there is no scar anyone can see. An open rhinoplasty adds one small incision under the nose, which usually heals to a fine line that is very hard to find.

What does “scarless” really mean?

No surgery reshapes bone and cartilage without an incision. In a scarless, or closed, rhinoplasty, the surgeon works entirely through openings made inside the nostrils. They heal like any incision does, but they are tucked out of sight, so the outside of the nose shows nothing.

So the honest version is this: scarless means no visible scar. It does not mean the nose was reshaped without surgery, and it does not mean recovery is skipped. The bridge, the bones and often the tip are still reshaped from within.

Every Rhinoplasty Has Incisions

“Scarless” means none of them can be seen. Here is where each one hides.

Closed and open

Inside the nostrils

Used in every rhinoplasty. They heal out of sight, so no one can see them.

Open only

Across the columella

A small incision on the strip of skin between the nostrils. It usually heals to a fine line that is hard to find.

Only if needed

At the nostril base

Added only when the nostrils need narrowing, in either approach, and placed in a hidden spot in the natural crease.

Where the incisions go in closed and open rhinoplasty

Both approaches share the same hidden incisions inside the nostrils. The difference is one small cut. In an open rhinoplasty, Dr. Naderi adds an incision across the columella, the narrow strip of skin between the nostrils, which lets him lift the skin and see the cartilage framework directly.

Closed (scarless)

No visible scar

Every incision sits inside the nostrils.

Open

A fine line under the nose

The same inner incisions, plus a small one across the columella.

Hidden inside the nostrilsSmall incision across the columella

If the nostrils are wide, either approach can add small incisions at the base of the nostrils to narrow them. Dr. Naderi places those in a hidden location along the natural crease, where they blend in as they heal.

How visible is the open rhinoplasty incision?

This is the part that worries people most, and it is usually the part that matters least. The columella incision is only a few millimeters long. Placed and closed with care, it tends to fade over the following months into a fine line that friends, coworkers and even partners rarely notice.

Don’t let a three-millimeter incision hidden under the nose keep you from a great nose job. I choose the approach that gives you the best result, and I will not compromise a result for marketing.

In practice, most people who have an open rhinoplasty with a careful surgeon forget the incision is there. What they do notice, every day, is the shape of their nose. That is why the shape should lead the decision.

Why “scarless” became a marketing word

Terms like closed, scarless and atraumatic rhinoplasty often lead patients to believe one method is simply better than the other. Very often those words are used for advertising. The truth is quieter: each nose is different, and the best approach is the one that fits that nose.

Dr. Shervin Naderi performs both. Earlier in his career, most of his noses were done closed. Today about 85 percent of his rhinoplasties are open and 15 percent are closed, because the open approach lets him see and support the tip with more precision. When a closed rhinoplasty is the best tool for a nose, he uses it, and he does it well.

Who is a good candidate for a scarless nose job?

Closed rhinoplasty tends to suit noses where most of the change is on the bridge. Lowering a dorsal hump, smoothing a small bump, narrowing the bones or making a subtle refinement while leaving a well-shaped tip alone are classic examples.

Noses that need more structural work usually do better open. Uneven tip cartilages, weak support, thick skin that needs a strong framework and noses that have had surgery before all benefit from the direct view the open approach gives. Caring for your incisions as they heal

Inside the nostrils, the incisions need gentle cleaning and patience. After an open rhinoplasty, the small incision under the nose is kept clean while it heals. Staying out of the sun, avoiding picking at crusts and following your post-operative instructions all help any incision fade at its best.

Your full routine is laid out in our rhinoplasty instructions, and our team walks you through it before surgery so nothing comes as a surprise.

What matters more than the scar

The questions that change your result are about the surgeon, not the incision. How many noses does he operate on? Do his results look natural from every angle? Does he plan each nose for the face it belongs to? Those answers decide how you will feel in a year.

Dr. Naderi is a double board-certified facial plastic surgeon with more than 20 years of practice and a strong surgical focus on rhinoplasty. Patients travel to his offices in Reston, Virginia and Chevy Chase, Maryland from Arlington, Alexandria, Vienna, Potomac and well beyond the DC area.

Frequently asked questions

Is scarless rhinoplasty completely scar-free?

Not technically. Every rhinoplasty uses incisions, but in a scarless (closed) rhinoplasty they all sit inside the nostrils, so no scar can be seen on the outside of the nose.

How big is the scar from open rhinoplasty?

The open approach adds one small incision, a few millimeters long, across the columella under the nose. With careful technique it usually heals to a fine line that is very hard to find.

Is closed rhinoplasty better than open rhinoplasty?

Neither is better for every nose. Closed suits many bridge and hump changes; open is usually better for tip work, weak or uneven cartilage and revisions. Dr. Naderi performs both and recommends the one that fits your nose. Learn more on our scarless rhinoplasty page.

Does Dr. Naderi perform scarless rhinoplasty?

Yes. Dr. Shervin Naderi performs both closed (scarless) and open rhinoplasty at The Naderi Center in Reston, VA and Chevy Chase, MD. About 15 percent of his rhinoplasties are closed.

Can nostril narrowing be done without a visible scar?

Nostril narrowing uses small incisions at the base of the nostrils. Dr. Naderi places them in a hidden location along the natural crease so they blend in as they heal.

How much does scarless rhinoplasty cost?

Scarless rhinoplasty is priced the same as rhinoplasty at The Naderi Center. Your exact fee is set at your consultation.
