What is Revision Rhinoplasty?
Revision rhinoplasty is a second or third operation to correct a nose after an earlier rhinoplasty. Some patients are unhappy with the shape. Others have not breathed well since their first surgery. Many have both problems. A revision can be small, such as smoothing a bump or refining the tip, or it can mean rebuilding the support of the nose.

What Revision Rhinoplasty Can Correct
Revision rhinoplasty can correct problems with both the look and the function of the nose, including:
- Trouble breathing, often from a collapsed nasal valve or a septum that is still crooked. See an example.

Why Revision Is Harder Than a First Rhinoplasty
Every surgery leaves scar tissue, and scar tissue does not move or heal like untouched tissue. The first surgery may have removed cartilage the nose needs for support, so that support has to be rebuilt. The skin may also be less forgiving than it was the first time. How to Choose a Revision Rhinoplasty Surgeon
A revision is your second chance at the nose you wanted, so the choice of surgeon matters even more the second time. Ask these questions:
- Is the surgeon board certified in facial plastic surgery? Dr. Naderi is certified by the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology–Head and Neck Surgery.
- Does the surgeon show revision results, not just first rhinoplasties? See Dr. Naderi’s revision before and after photos.
- Can the surgeon rebuild support when the septum is gone? Dr. Naderi prefers to use your own rib cartilage.
- Can the surgeon fix your breathing in the same operation?
- Does the surgeon do the computer imaging personally? Dr. Naderi does his own imaging, and he will tell you when scar tissue makes the result too hard to predict for imaging to be accurate.
- Will the surgeon tell you when another surgery is not a good idea? Dr. Naderi advises against surgery when it is not in your best interest.

How Long Should I Wait Before a Revision?
Dr. Naderi asks patients to wait one year after their last nose surgery. Swelling has to settle and scar tissue has to mature before the result can be judged fairly and the next surgery planned safely.
Rebuilding the Nose with Cartilage Grafts
Many revision noses need cartilage grafts to restore support. When enough is left, Dr. Naderi uses cartilage from your own septum.
When the septum was used up in the first surgery, Dr. Naderi prefers to use your own rib cartilage. It is strong, there is plenty of it, and it is your own tissue. It is taken through a small incision on the chest, and you still go home the same day. When your own rib is not a good option, he uses donor rib cartilage.
Who Is a Good Candidate for Revision Rhinoplasty?
You may be a good candidate for revision rhinoplasty if:
- your last nose surgery was at least a year ago
- you are unhappy with how your nose looks, how it breathes, or both
- you are in good health and willing to stop smoking before and after surgery
- you have realistic goals
Dr. Naderi will tell you honestly if another surgery is not in your best interest.
Your Revision Rhinoplasty Consultation and Computer Imaging
At your consultation, Dr. Naderi examines the outside and the inside of your nose, asks what bothers you about it and how you breathe, and reviews what was done before.
He does the computer imaging himself, so what you see on the screen is surgically realistic. In some revision cases, scar tissue from earlier surgery makes the final result too hard to predict. When that happens, Dr. Naderi may choose not to do imaging rather than show you a picture that may not be accurate.

Insurance. Medical insurance does not pay for cosmetic revision rhinoplasty. It may cover part of the surgery when there is a medical need, such as correcting a breathing problem.

How Revision Rhinoplasty Is Performed
Revision rhinoplasty is done under general anesthesia, with a board-certified anesthesiologist with you from start to finish. Most revisions are done open, through a small incision across the skin between the nostrils, so Dr. Naderi can see and rebuild the structures directly. Surgery usually takes two to four hours. Breathing problems are corrected in the same operation.

Revision Rhinoplasty Recovery, Week by Week
Recovery after a revision is the same as after a first rhinoplasty. Plan on one week off work or school. A small cast stays on your nose for 5 to 10 days, and exercise waits for one month.

What Are the Risks of Revision Rhinoplasty?
Every surgery has risks. The main ones are bleeding, infection, swelling that takes time to settle, breathing that is not fully improved, asymmetry, and the chance that another small procedure is needed. When your own rib cartilage is used, there is a small scar on the chest and some soreness there while it heals. Dr. Naderi goes over the risks that apply to your nose at your consultation.
