Surgery Preparation Checklist

Complete Naderi Center surgery scheduling forms

Confirm dates/times of all appointments

You will have received emails as well as text confirmations of any necessary preoperative and post-operative appointments. Please contact your patient coordinator if you have any questions or need to make changes to these dates.

Complete preoperative medical evaluation and testing

Physician testing prescriptions as well as laboratory testing prescriptions will have been emailed to you. Complete all testing no sooner than 30 days prior to surgery. We recommend all medical work up be performed at least 2 weeks prior to surgery.

Pay remaining balance of Naderi Center’s surgeon fee

Surgery balance payments are due at time of your preoperative appointment, or no later than 3 weeks prior to surgery. Payments by personal checks or bank checks are preferred.

PAYMENT BY CHECK OR BANK CHECK

We advise check payments to be submitted in person. If you are unable to submit payment in person please inquire on how payments can be submitted electronically.

FINANCING OPTIONS

The Naderi Center participates with a number of reputable financing programs, information as well as applications can be found here.

CREDIT CARD PAYMENTS

A 3 % credit card processing fee will be applied to surgery payments made with credit card. Discovery, Visa and Mastercard accepted.

Complete surgical facility registration and payment for facility and anesthesia fees

Please select your surgical facility to complete any necessary facility registration as well as make payment for anesthesia and facility fees.

Review preoperative forms and consents

Start recommended pre-surgical diet

Two weeks prior to surgery it is recommended that you refrain from certain foods, supplements and medications to minimize post-operative bruising and swelling.
