One of the first things people hear about closed rhinoplasty is that the recovery is easier. It is an understandable assumption: no incision under the nose must mean less healing. The real difference is smaller than most people expect, and knowing what to plan for makes the whole experience calmer.
Here’s your guide to recovering from closed and open rhinoplasty, what is the same, what is a little different, and what each week tends to look like.
The short answer: the milestones are almost identical. Either way, the cast comes off in five to ten days, most people take a week off work and exercise waits a month. Closed rhinoplasty may have slightly less early tip swelling, and there is no outside incision to care for.
Your recovery, week by week
Your Rhinoplasty Recovery
Closed or open, the milestones are much the same
Rest with your head raised. Some stuffiness is normal.
The cast stays on 5 to 10 days. Plan on a week off work.
Most people are back to dinners and meetings.
Workouts can start again after one month.
The shape firms up and the nose feels like your own.
The skin slowly shrink-wraps, so the tip keeps gaining definition.
The first week
You go home the same day as surgery. The first few days are about rest, keeping your head raised and letting the swelling settle. Stuffiness is normal, since the inside of the nose is healing too. Most patients describe the discomfort as pressure more than pain.
The cast stays on for five to ten days. Plan on a full week away from work, and do not count on returning after two or three days. Your detailed routine for this week is in our rhinoplasty instructions.
Weeks two and three
With the cast off, you will see your new nose for the first time. It will look swollen, especially at the tip, but the change is usually clear. Any bruising fades over these weeks and can be covered with makeup, and most people are comfortable at dinners, errands and meetings again.

One month and beyond
At one month, exercise can begin again. Over the next few weeks the nose starts to feel natural, and by about three months the shape has firmed up enough that most people stop thinking about it day to day.
The tip is the slowest part. As swelling leaves and the skin shrink-wraps around the new framework, the tip keeps gaining definition for three to five years, especially with thicker skin. That gradual change is a good thing: it is how a result comes to look like it was always yours.
What is actually different about closed recovery
Closed vs. Open Recovery
What is the same, and what is a little different
| Closed | Open | |
|---|---|---|
| Outside incision | None | A small one under the nose to keep clean |
| Early tip swelling | Often a little less | Often a little more, since the tip skin is lifted |
| Time off work | About a week | About a week |
| Cast | 5 to 10 days | 5 to 10 days |
| Exercise | After one month | After one month |
| Final result | Keeps refining for years | Keeps refining for years |
In a closed rhinoplasty, the skin over the tip is not lifted, so early tip swelling is often a little less. There is also no small incision under the nose to keep clean. Those are real advantages, but they are modest, and they do not change the time off work, the cast or the exercise rules.
Recovery is not the reason to choose closed over open. The reason is your nose. When the tip needs real work, the open approach gives a better result, and the extra swelling settles.

Tips that help either way
Sleeping with your head raised, staying cool and resting more than you think you need all help early on. For thicker skin, taping the nose after surgery can keep swelling down. Sun protection protects the healing skin, and patience protects your peace of mind.
If you are traveling for surgery, plan to stay about a week. Patients flying in from out of town have our nursing concierge service during recovery, and car service to and from surgery can be arranged when needed.
Planning around your calendar
For most people in Reston, Vienna, Arlington, Alexandria, Bethesda, Chevy Chase and Washington, DC, a week off plus a few weeks of light social plans is enough. If you have a wedding or big event, leave a few months between surgery and the date so the swelling has time to settle.
Dr. Shervin Naderi is a double board-certified facial plastic surgeon with more than 20 years of practice and a strong surgical focus on rhinoplasty. He will map out your recovery at your consultation in Reston or Chevy Chase.

Frequently asked questions
Is recovery faster after closed rhinoplasty?
Only slightly. Closed rhinoplasty may have a little less early tip swelling and no outside incision, but the cast, time off work and exercise rules are the same as open rhinoplasty.
How long do I need off work after rhinoplasty?
Plan on a full week off work after closed or open rhinoplasty.
When does the cast come off after rhinoplasty?
Usually five to ten days after surgery.
When can I exercise after rhinoplasty?
Exercise can resume after one month.
How long until my nose looks final?
The nose looks natural within a few months and feels like your own by about three months, but the tip keeps refining for three to five years as the skin shrink-wraps.
Do I stay overnight after rhinoplasty?
No. Rhinoplasty with Dr. Naderi is done under general anesthesia and patients go home the same day.
