Cheek Filler in the Washington DC Area: Reston, VA and Chevy Chase, MD

Cheek filler at The Naderi Center is performed only by physicians: Dr. Shervin Naderi, a double board-certified facial plastic surgeon, and Dr. Alexandra Snodgrass, a board-certified dermatologist, at our offices in Reston, Virginia and Chevy Chase, Maryland.

Filler placed in the right part of the cheek restores lost volume, supports the midface and softens tired shadows under the eyes, for a rested, natural look. Most patients need 2 to 4 syringes of Juvederm Voluma, Restylane Lyft, Restylane Contour or RHA 4.

What Cheek Filler Can Do

Cheek filler restores the volume the face loses with time, so you look rested and healthy rather than tired. Done well, no one should be able to tell.

Fat-pads are located below the skin’s surface and help to provide volume, facial contours, and fullness. As we age, fat-pads get thinner and descend. As a result, our face may not look as round and firm as it did in our youth. Hollows can form beneath the eyes, drooping skin can cause deeper lines around the nose and mouth, loose skin can result in a sagging jawline, and fat can accumulate beneath the chin, resulting in fullness between the neck and chin (“double chin”). The loss and downward movement of fat-pads can also make the face appear deflated and sunken in the cheek area.

What Is Cheek Filler?

Cheek filler is an injection of hyaluronic acid, a sugar molecule your body already makes, prepared as a firm gel and placed deep along the cheekbone and in the midface. Cheek fillers are thicker than lip fillers, so they hold their shape and support the tissue above them, and they contain lidocaine to keep the treatment comfortable.

Hyaluronic acid filler is not permanent, and it can be dissolved with an enzyme called hyaluronidase if it is ever needed. Sculptra is a different option that gradually rebuilds your own collagen over a few months.

Where Does Cheek Filler Go?

In women, filler is placed in the front of the cheek, beside the smile lines, to fill a flat or sunken midface and soften the hollow between the lower eyelid and the cheek. A little more along the cheekbone, below the outer corner of the eye, adds contour. The goal is a smooth S-shaped curve, called the ogee curve, when the face is turned slightly to the side.

In men, the goal is different. Filler projects and strengthens the cheekbone for a chiseled, masculine midface, without adding the soft fullness in the front of the cheek that reads as feminine.

The midface comes first. Many patients who ask about dark circles, smile lines or early jowls need volume in the midface first. Restoring the cheek often improves all three. Learn more about filler for men and under-eye hollows.

How Dr. Naderi Approaches Cheek Filler

A common mistake is placing filler only high on the cheekbone. Without volume in the midface below it, the face can look more hollow and skeletal, not younger. We start where the face has lost volume and build from there.

“When this area fills up, it just looks healthier, or rested, more youthful.”

Dr. Shervin Naderi

How we inject cheeks

  • Right on the bone. Filler is placed deep, straight down onto the cheekbone, where there are very few blood vessels. This placement is precise and lowers risk.
  • Enough filler to make a difference. A full correction looks better and lasts longer than a little filler spread thin. Most patients need 2 to 4 syringes.
  • Never overfilled. Filler cannot lift sagging skin without overfilling the face, which looks unnatural. When a facelift would do more, we tell you.
  • Refilled before it wears off. Periodic touch-ups keep the result steady, so the face never goes back to where it started.

“It’s like asking how long a haircut lasts… You surely don’t want to wait for all the filler to go away.”

Dr. Shervin Naderi

“Cheek injections are the new lip injection. More and more patients are realizing that fillers to the mid-face can restore youthfulness, and in the younger population we are seeing a huge demand for the modelesque high cheek bone look.”

Dr. Alexandra Snodgrass

Who Is a Good Candidate for Cheek Filler?

Cheek filler is a good fit if you have:

  • Flat, sunken or deflated cheeks
  • Midface volume loss with age or after weight loss, including on Zepbound or Ozempic
  • Shadows or hollows under the eyes that come from a flat midface
  • Deep smile lines that start up near the nose
  • A wish for higher, more defined cheekbones

We will suggest something else when:

  • Skin is loose and sagging along the jaw and neck. A deep plane facelift lifts it; filler cannot without overfilling.
  • A facelift is planned within about a year. We usually skip filler and add fat injections during surgery instead.
  • You prefer a gradual change. Sculptra builds your own collagen over a few months.
  • You are pregnant or breastfeeding, or have an active skin infection. We will wait.

What Happens During a Cheek Filler Appointment?

Cheek filler is done in the office, usually in a single visit. Plan on 20 to 30 minutes for numbing cream before your injections.

Which Cheek Fillers Do We Use?

Your physician chooses the filler for your face and your goal, and many patients do well with more than one. Every hyaluronic acid filler we use can be dissolved if needed.

Juvederm Voluma XC cheek filler box

Juvederm Voluma XC

Volume and lift

The first filler FDA approved for the cheeks. A firm gel that holds its shape, often chosen to restore midface volume, with results that can last up to 2 years.

Restylane Lyft

Lift and structure

Formerly called Perlane. A firm, supportive gel FDA approved for cheek augmentation and midface volume loss.

Restylane Contour

Contour that moves

A flexible gel designed to move naturally with your expressions, FDA approved for cheek augmentation and midface contour.

RHA 4

Deep, dynamic volume

The firmest filler in the RHA collection, designed to stretch and move with your expressions for deeper volume and contour.

Sculptra

Gradual collagen building

Poly-L-lactic acid that stimulates your own collagen. Results build over a few months, usually after 2 to 3 sessions spaced 4 to 6 weeks apart. Unlike hyaluronic acid filler, it cannot be dissolved.

Prefer your own tissue? Fat injections are done by Dr. Naderi in the operating room, often with a facelift. Learn more about Sculptra, RHA fillers and all of our dermal fillers.

What Does the Research Say About Cheek Filler?

Hyaluronic acid cheek filler has been studied for more than a decade. Two studies worth knowing:

Patients felt they looked about 5 years younger

In a randomized, controlled study of Juvederm Voluma XC for age-related midface volume loss, about 93% of patients rated their cheeks as improved at 6 months, and 79% still did at 2 years. On average, patients said they looked 5 years younger at 6 months and 3 years younger at 2 years.

Restylane Lyft results held for at least a year

In a randomized, evaluator-blinded study of 200 patients, 89% of those treated with Restylane Lyft had a visible improvement in midface fullness at 8 weeks, compared with 16% of untreated patients. Results stayed significantly better than no treatment through 12 months, with a very good safety profile.

How to Prepare for Cheek Filler

For 2 weeks before, avoid nuts, seeds, fish and fish oil, aspirin, ibuprofen, naproxen and supplements that thin the blood, such as vitamin E. This lowers your chance of bruising. If you take a prescription blood thinner, ask the doctor who prescribed it; never stop a medicine you need for a cosmetic treatment. See our full Botox and filler instructions.

If you get cold sores, tell us before your visit. Your physician will prescribe an antiviral medicine to start before your injections.

Arnica, started a few days before, may help bruises fade faster.

On the day, eat and drink normally, come with little or no makeup, and arrive about 30 minutes early for numbing cream.

Have an event coming up? Plan your cheek filler at least 2 weeks before, so any swelling or bruising has time to settle.

What Is Recovery Like After Cheek Filler?

Most people go back to work and their normal routine the same day. Expect some swelling and tenderness, and possibly a bruise, for the first several days.

Cheek filler recovery timeline
WhenWhat to expect
Right afterThe cheeks can feel firm and a little tender, and small pinpoint marks are normal. Ice gently, 20 minutes on and 20 minutes off.
First 48 hoursNo strenuous exercise. Sleep with your head raised. Do not massage or press on the cheeks unless your physician asks you to.
First weekSwelling settles and any bruises begin to fade. Makeup can cover a bruise the same day.
Two weeksThis is your result. Firmness and small bumps from swelling usually resolve by now. If you want more volume, this is the right time for a touch-up.

For pain, use Tylenol, not ibuprofen. Call us right away for severe or worsening pain, skin that turns white, gray, blue or blotchy, or any change in your vision.

Is Cheek Filler Safe? Risks and Who Should Wait

Common and short-lived

  • Swelling and tenderness for several days
  • Bruising, which can last 1 to 2 weeks
  • Firmness or small bumps while the swelling settles, usually gone within 2 weeks
  • Sides that look uneven while they are swollen
  • A cold sore flare in people who get them

Rare but serious. Filler can block a blood vessel. Signs include severe pain, skin that turns white, gray or blotchy, and changes in vision. Injecting straight onto the bone, where there are few vessels, lowers this risk, and hyaluronic acid filler can be dissolved right away if it is ever needed.

Who should wait

  • Pregnant or breastfeeding
  • An active skin infection or cold sore
  • An important event in less than 2 weeks
  • A facelift planned within about a year

Why filler can look “migrated.” Filler does not travel to another part of the face. When cheeks are overfilled, the face looks puffy and heavy. Using the right amount in the right place prevents it, and hyaluronic acid filler can be dissolved and corrected.

Cheek Filler FAQs

Cheek filler costs $900 per syringe, the same price whichever filler you choose: Juvederm Voluma XC, Restylane Lyft, Restylane Contour or RHA 4. Most patients need 2 to 4 syringes, or $1,800 to $3,600. Sculptra is $1,000 per vial.

Most patients need 2 to 4 syringes, depending on how much volume the face has lost and the look you want. Dr. Naderi favors a full correction over a little filler spread thin, because it looks better and lasts longer. Spread across both cheeks, even 3 or 4 syringes look natural.

Only physicians: Dr. Shervin Naderi, a double board-certified facial plastic surgeon, and Dr. Alexandra Snodgrass, a board-certified dermatologist. Both inject cheek filler at our Reston, Virginia and Chevy Chase, Maryland offices.

It depends on the filler and how much is used. In studies, Juvederm Voluma results lasted up to 2 years and Restylane Lyft at least 12 months. In the cheeks, more filler tends to last longer. Dr. Naderi recommends periodic touch-ups before it wears off, so your face never goes back to where it started.

Most patients feel pressure more than pain. Numbing cream goes on for 20 to 30 minutes first, and cheek fillers contain lidocaine. Some people notice a soft crunching sound during the injections, which is normal.

There is no single best filler. Juvederm Voluma, Restylane Lyft, Restylane Contour and RHA 4 are all made for deep volume and contour, and each has strengths. Sculptra is an option for gradual change. Your physician chooses the filler based on your tissue, your anatomy and your goal.

Both are firm hyaluronic acid fillers FDA approved for the cheeks, and both can be dissolved. Voluma was the first filler approved for the cheeks and has studies showing results up to 2 years. Restylane Lyft, formerly Perlane, gives firm support and structure. Many patients do well with either.

Filler in the midface can give a subtle lift and soften the transition into the jowls, but it cannot lift truly sagging skin without overfilling the face, which looks unnatural. For loose skin along the jaw and neck, a deep plane facelift is the right answer, and Dr. Naderi will tell you honestly which one you need.

Yes, when it is placed in the midface where the volume was lost, not just high on the cheekbone, and when the face is not overfilled. The goal is that people notice you look rested and healthy, not that you had filler.

Often, yes. Many under-eye shadows come from a flat midface below them, and restoring cheek volume softens the hollow without putting filler directly under the eye. Learn more about treating dark circles and under-eye hollows.

Hyaluronic acid fillers such as Voluma, Restylane Lyft, Restylane Contour and RHA 4 can be dissolved with an enzyme called hyaluronidase. Sculptra cannot be dissolved, which is one reason we discuss it carefully before treatment.

Most people return to work the same day. Swelling and tenderness last a few days, and a bruise, if you get one, fades over 1 to 2 weeks. Avoid strenuous exercise for 48 hours and facials or face massage for a week.

Right away. The result settles over about 2 weeks as the swelling goes down, and that is the best time to judge it. Sculptra is different: results build gradually over a few months.

Filler gives an immediate result and can be dissolved. Sculptra builds your own collagen gradually over 2 to 3 sessions and cannot be dissolved. Some patients choose filler for an immediate change and Sculptra for long-term support.

Filler is done in the office with little downtime. Fat transfer uses your own fat, is done by Dr. Naderi in the operating room under general anesthesia and is often combined with a facelift. Part of the fat may remain long term. If you plan a facelift within about a year, we usually add fat during surgery instead of filler.

In experienced hands, cheek filler is very safe. The most important factor is the injector. Our physicians place filler deep along the bone, where there are few blood vessels, and hyaluronic acid filler can be dissolved if it is ever needed.

Yes. For men, filler projects and strengthens the cheekbone for a more chiseled midface, without adding the soft fullness in the front of the cheek that looks feminine. Learn more about dermal fillers for men.

Yes. Weight loss often takes volume from the cheeks and temples. Many patients wait until their weight levels off, so further loss does not undo the result. Read our guide to preventing Ozempic face.

Yes. Cheek filler is often done in the same visit as lip filler, chin filler, jawline contouring or Botox. Your physician will plan the order and the amounts with you.

Yes. If filler from another office looks puffy, lumpy or overfilled, we can dissolve it, starting at $1,500, and then restore the cheeks with our fillers at $900 per syringe. Learn more about fixing botched filler.
