This very pleasant young male came to The Naderi Center for evaluation after sustaining multiple nasal injuries from wrestling, which left him with both functional and cosmetic concerns. He reported longstanding difficulty breathing through his nose, with obstruction that fluctuated but was consistently worse on the left side.
On examination by Dr. Naderi, the patient was found to have thick skin. His nose presented with a mild dorsal hump, primarily cartilaginous in nature, along with a slightly under projected and under-rotated nasal tip. Internally, the septum was thick and curved significantly to the right, though paradoxically, the left nasal passage was more open. Externally, the nose slanted to the left, and both nostrils were narrow, with the right side more compromised due to septal deviation.
Using computer imaging, Dr. Naderi developed a surgical plan tailored to the patient’s anatomy and goals. Rather than aggressively reducing the dorsal hump, the focus was on tip projection and rotation to create better dorsal balance. Minor supratip refinement, septoplasty, osteotomies, and septal extension grafting were included to improve both aesthetics and function.
