Eyelid Surgery in the Washington DC Area: Reston, VA and Chevy Chase, MD

Dr. Shervin Naderi, a double board-certified facial plastic surgeon, performs eyelid surgery (blepharoplasty) for patients of The Naderi Center in Reston, Virginia and Chevy Chase, Maryland. Upper eyelid surgery removes heavy, hooded skin. Lower eyelid surgery removes under-eye bags through an incision hidden inside the lid, often with fat grafting to smooth the hollow beneath them.

Surgery is done in the operating room under general anesthesia. Upper eyelids take 60 to 90 minutes, and upper and lower together about two hours. Most patients take 1 to 2 weeks off work. Patients come to us from across Northern Virginia, Washington DC and Maryland, including McLean, Great Falls, Vienna, Arlington, Bethesda and Potomac.

Your Eyelid Surgeon: Dr. Shervin Naderi

Dr. Shervin Naderi, MD, FACS, is a double board-certified facial plastic surgeon and the founder of The Naderi Center in Reston, Virginia and Chevy Chase, Maryland, with more than 20 years in practice and more than 100,000 procedures performed. He operates only on the face and neck, and his goal is eyes that look rested and natural, never “done.”

Honest advice: Dr. Naderi also performs his own injectable treatments, so his advice isn’t skewed toward surgery. If filler or another non-surgical option will give you what you want, he’ll tell you, and if it won’t, he’ll tell you that too.

What Can Eyelid Surgery Fix?

Eyelid surgery corrects the changes that make you look tired when you aren’t: heavy, hooded upper lids and bags under the eyes. The goal is a rested version of your own eyes, not a new shape.

Upper Eyelid Surgery

As eyelid skin loosens with age, it folds over the lid and makes you look older and tired. Many people raise their brows without realizing it to hold their eyes open, which deepens forehead lines. When heavy upper lids are the real problem, Botox in the forehead can make them feel even heavier, because it stops you from lifting your brows. That is why Dr. Naderi checks whether heavy lids are the cause before treating forehead lines. Sometimes the answer is a brow lift instead of, or along with, eyelid surgery.

Lower Eyelid Surgery

Under-eye skin is the thinnest on the face, so it is one of the first places to show age. Bags form when the fat cushion around the eye bulges forward, often with a hollow beneath it. Injections under the eyes can soften a hollow, but they cannot remove bags. Dr. Naderi removes the bulging fat through a transconjunctival (inside-the-lid) incision, so the scar is hidden from view. He often adds fat grafting, using a small amount of your own fat, to create a smoother transition from under the eyes to the cheeks, which can give an even better result. Repositioning the eyelid fat is saved mainly for patients who don’t have fat to spare elsewhere.

Dr. Naderi’s Approach: Less Skin, More Volume

Many surgeons still remove a lot of eyelid skin. That older technique tightens the lids, but it can leave people looking older and sick instead of rested. Dr. Naderi is more conservative: he removes skin only where it truly needs to be removed and restores the volume your eyes have lost with age.

Conservative Skin Removal, Restored Volume

For women and men alike, the trend today is toward removing less skin. With age the eyes lose volume, not just gain loose skin, so Dr. Naderi often adds fat grafting with your own fat.

Fat is not only for the mid face and tear troughs. He also places it along the upper eyelid and the bony rim under the brow to soften the hollow, “A-frame” look of aging upper lids.

The Brow Comes First

In Dr. Naderi’s experience, about 9 out of 10 older patients who come in for upper eyelid surgery also need their brow reset. If the brow is low and only skin is removed, the brow can be pulled down further, the eyes look worse, and the hooding at the outer corners stays.

That is why he often pairs upper eyelid surgery with an endoscopic-style brow reset.

Fat Placed With Precision

Fat grafting is only as good as the technique. Injected at the wrong angle, fat can form a lumpy, sausage-like ridge in the upper or lower lids, a complication that is very hard to correct afterward.

Dr. Naderi places fat perpendicular to the area being filled, so it sits smoothly where it belongs.

Are You a Good Candidate for Eyelid Surgery?

Good candidates are healthy adults whose eyelids make them look tired or older than they feel. At your consultation, Dr. Naderi examines your eyelids, brows and cheeks together to decide what will actually help.

Upper eyelid surgery may be right for you if:

  • Extra skin folds over your eyelid crease
  • Your lids look heavy or hooded
  • Eyeshadow disappears into the fold or smudges
  • There’s puffiness near the inner corner of the upper lid

Lower eyelid surgery may be right for you if:

  • You have under-eye bags that don’t go away with sleep or creams
  • Your bags cast a shadow that looks like dark circles
  • Puffiness under your eyes makes you look tired even when you’re rested

Who Should Wait on Eyelid Surgery

Eyelid surgery is elective, so the timing should be right. Dr. Naderi may recommend waiting if you:

  • Are pregnant or breastfeeding.
  • Have a medical condition that isn’t under control yet. It should be well managed before any elective surgery.
  • Have an eye infection or another eye problem that needs to be treated first.
  • Had LASIK or other eye surgery recently. Your eyes should be fully healed first.
  • Smoke or use nicotine. Nicotine slows healing, and all nicotine has to stop at least two weeks before surgery.
  • Want a different look, not a refreshed one. The goal is eyes that look rested and natural, never done.

Eyelid Surgery or a Brow Lift?

Sometimes the heaviness above the eyes comes from a brow that has dropped, not from the eyelid. Removing eyelid skin alone won’t fix a low brow. Dr. Naderi checks your brow position at your consultation. Some patients do best with a brow lift, and some with both procedures together.

Eyelid Surgery or Under-Eye Filler?

Dr. Naderi steers most patients away from tear trough filler. In thin under-eye skin it can cause swelling and a bluish-gray bag that can last for years. For patients who need only a minimal improvement, PRF gel injections are a non-surgical option. For a real hollow under the eyes, fat injections are the better choice. Neither one removes bags. When the bags themselves are the problem, lower eyelid surgery is the lasting fix.

Eyelid Surgery for Men

Eyelid and brow surgery are different for men and women. For men, the key is being conservative and replenishing volume.

  • Skin removed only where it needs to be. Taking too much makes the lids look tight and the eyes look older, not rested.
  • A lower, flatter upper lid crease, which looks natural on a man.
  • Fat added along the upper eyelid and the rim under the brow, which gives a much more natural result than removing skin alone.
  • A flatter brow. When the brow needs to be reset, Dr. Naderi avoids creating too much of an arch, so a man’s brows are never feminized.

Recovery is the same as for women.

What to Expect on Surgery Day

Upper and lower eyelid surgery can be done on their own or together in the same surgery. Here is what happens, from two weeks before surgery to the ride home.

  1. Getting Ready, Starting 2 Weeks Before

    • Your prescriptions are sent to your pharmacy at least two weeks ahead.
    • Start Dr. Naderi’s anti-bleeding, anti-bruising diet. Stop aspirin, ibuprofen, alcohol and blood-thinning supplements. If a doctor prescribed a blood thinner, check with that doctor before stopping it.
    • Stop all nicotine at least two weeks before surgery. Four weeks is better.
    • Nothing to eat or drink after midnight the night before.

  2. In the Operating Room

Dr. Naderi performs eyelid surgery under general anesthesia in the operating room at Prince William Surgery Center, where he is medical director. Anesthesia is given by board-certified anesthesiologists and the nurse anesthetists (CRNAs) they supervise.

    • Upper eyelids (60 to 90 minutes): Dr. Naderi measures and marks your lids so both creases match. He then removes the extra skin, and a little fat if needed, through an incision in your natural crease. With your eyes open, the scar is hidden in the fold.
    • Lower eyelids: through a transconjunctival incision inside the lower lid, Dr. Naderi removes the fat that causes the bags. He often adds fat grafting, using your own fat, to smooth the hollow beneath them; repositioning the eyelid fat is mainly for patients who don’t have fat to spare elsewhere. There’s no cut on the skin of the lower lid.
    • Upper and lower together take about two hours.
  3. Going Home

You go home the same day. A responsible adult needs to drive you and stay with you at first; you can’t be released to an Uber or taxi. Stitches in the upper lids come out about a week after surgery.

Eyelid Surgery Risks and Safety

Serious problems after eyelid surgery are rare. Dr. Naderi goes over every risk with you at your consultation.

Possible Risks

  • Bleeding behind the eye (retrobulbar hemorrhage). It’s rare, but it can threaten vision, which is why sudden pain, swelling or vision loss means calling 911.
  • Infection
  • Dry or irritated eyes for a while after surgery
  • Double vision, usually temporary
  • Visible scarring
  • Eyelid position problems, such as the lower lid pulling down
  • Too much skin or fat removed, leaving a hollow look
  • Differences between the two sides
  • Numbness of the eyelids, usually temporary

How Dr. Naderi Lowers Your Risk

  • Precise measurements, so both creases match.
  • Conservative skin removal. Only the skin that needs to go is removed, so the lids don’t end up tight or hollow.
  • Lower lids done through a transconjunctival (inside-the-lid) incision, with no cut on the skin. This approach is less likely to pull the lower lid down.
  • No hollow, “surprised” look. He often adds fat grafting, using your own fat, to smooth the area under the eyes, which can give an even better result. Repositioning the eyelid fat is saved mainly for patients who don’t have fat to spare elsewhere.
  • Fat placed perpendicular to the area being filled, to avoid lumpy, sausage-like ridges.
  • Two weeks of preparation: his anti-bleeding diet, no aspirin, ibuprofen or blood-thinning supplements (a prescribed blood thinner only with your doctor’s OK), and no nicotine.
  • Clear recovery instructions, and a team you can reach any time with photos or a call.

Eyelid Surgery (Blepharoplasty) FAQs

At The Naderi Center, upper eyelid surgery is $8,000 to $15,000 and lower eyelid surgery is $8,000 to $15,000, including Dr. Naderi’s fee, general anesthesia, the surgical facility fee and all follow-up visits. Cherry financing is available.

No. Eyelid surgery at The Naderi Center is cosmetic, so it is self-pay and is not billed to insurance. Cherry financing is available if you would like to pay over time.

Most patients are back at work in 1 to 2 weeks and look presentable in about 10 to 14 days. No strenuous exercise for a month, full activity by about 3 months, and final results at 3 to 6 months.

You can drive once you are no longer taking the prescription pain medicine and there isn’t much swelling left on your eyelids. On the day of surgery, a responsible adult needs to drive you home.

Most patients describe it as mild. You’ll go home with prescription pain medicine, and you can use Tylenol instead if the pain is mild.

Dr. Naderi prefers that you are asleep, so you stay completely still and he has the best possible conditions for precise work around your eyes. Many patients also have eyelid surgery together with other procedures, and general anesthesia covers all of them in one surgery. Anesthesia is given by board-certified anesthesiologists and the nurse anesthetists (CRNAs) they supervise.

Yes. Together they take about two hours, and you have one recovery instead of two.

Upper eyelid scars sit in your natural crease and are hidden when your eyes are open. For lower eyelids, the incision is made inside the lid (a transconjunctival incision), so there is no visible scar on the skin.

Upper eyelid results typically last 10 years or more, and lower eyelid surgery rarely needs to be repeated, because fat that is removed does not come back. Your eyes will keep aging naturally.

In Dr. Naderi’s experience, his eyelid surgery patients don’t regret it. The result patients most want to avoid is lids that look tight or hollow because too much skin was removed. That is why Dr. Naderi is conservative: he removes only the skin that needs to go and restores lost volume with your own fat, so your eyes look rested and still look like you.

With age, eyelid skin loses collagen and elasticity and begins to fold over the lid. Under the eyes, the fat cushion around the eye pushes forward and forms bags, often with a hollow beneath. Genetics plays a big part, which is why some people have bags in their 20s or 30s.

There is no single best age. Many of Dr. Naderi’s eyelid patients are in their 40s, when heavy upper lids and under-eye bags start to show, and 70 is not too old if you are healthy. Your overall health matters more than your age, and Dr. Naderi checks your brows and cheeks along with your eyelids before recommending surgery.

No. Ptosis is a drooping of the eyelid itself, caused by a weak muscle that lifts the lid, and it is different from extra skin. Dr. Naderi does not repair ptosis. He checks your brow and eyelid position at your consultation, and if your eyelid itself droops, he will tell you, because removing skin alone would not fix it.

Eyelid surgery removes extra skin and bags, but it does not erase fine lines or crepey skin. For those, laser resurfacing around the eyes can be done during the same surgery, and Botox can soften crow’s feet.

Many patients combine eyelid surgery with laser resurfacing around the eyes, a brow lift, a facelift, fat grafting or rhinoplasty.

Usually, yes. Tell Dr. Naderi at your consultation. He is especially careful about how much upper lid skin he removes, because removing too much can leave more of the eye exposed and make dryness worse.

Not until Dr. Naderi clears you. You can wear your glasses right after surgery.

Dr. Shervin Naderi, a double board-certified facial plastic surgeon. He performs eyelid surgery under general anesthesia at Prince William Surgery Center, where he has been medical director since 2023.

Both facial plastic surgeons and oculoplastic surgeons are trained to operate on the eyelids. Dr. Naderi is a double board-certified facial plastic surgeon whose training covers the whole face, so he looks at your eyelids together with your brows and cheeks. That often decides whether eyelid surgery alone will give you the result you want, or whether a brow reset or fat grafting should be part of the plan. Whoever you see, ask about their board certification and how often they perform eyelid surgery.

He is conservative with skin. He removes only the skin that needs to go and restores lost volume with your own fat, including along the upper eyelid and the rim under the brow. He also checks your brow first, because, in his experience, about 9 out of 10 older patients who come in for upper eyelid surgery also need a brow reset. Every lower lid is done through a transconjunctival (inside-the-lid) incision, with no cut on the skin.

Yes. Dr. Naderi is more conservative with men. He removes skin only where it needs to be removed, keeps the upper lid crease lower and flatter, and often adds fat along the upper eyelid and the rim under the brow. If the brow needs to be reset, he avoids creating too much of an arch, so a man’s brows are never feminized.
