Of all the questions we hear after lip filler, the most common is a gentle, slightly anxious one: are my lips supposed to look like this? Swelling is a normal part of the first few days, and knowing what each day usually brings makes the whole experience calmer. Here is your guide to lip filler swelling, day by day, from the moment you leave our office to the morning your result is settled, with what helps along the way.
How long does lip filler swelling last?
The short answer: lip filler swelling is temporary and follows a gentle, predictable pattern. Most of it settles within about one week, and your final result shows at two weeks. Most people return to work and their usual routine the same day.

Lips swell more readily than most of the face. The filler we use is a hyaluronic acid gel that holds water, and the lips are rich in small blood vessels, so they respond quickly to even very fine injections. “Swelling after lip filler is your body doing exactly what it should,” says Dr. Shervin Naderi, double board-certified facial plastic surgeon. “What we want is for you to know what to expect, so the first morning feels familiar instead of surprising.”
What do my lips look like right after lip filler?
Right after treatment, your lips feel numb for an hour or two and look a little fuller than they will end up. Small pinpoint marks where the filler went in are normal and fade quickly. You will see your new shape in the mirror before you leave, and we suggest icing gently at home, 20 minutes on and 20 minutes off.
How the filler is placed makes a real difference to how the first days feel. At The Naderi Center, every lip treatment is performed by a physician, either Dr. Naderi or board-certified dermatologist Dr. Alexandra Snodgrass, using many tiny micro-droplets rather than a few large deposits. “We examine your lips first and leave the areas that are already full alone,” Dr. Naderi says. “Small, careful amounts give a smoother result, with less need for massage afterward.”

Why do my lips look biggest the morning after?
The next morning is when swelling is usually at its peak, and it is the moment patients are most likely to call. Lips can look uneven or feel a little firm while they are swollen, and one side may look fuller than the other. Sleeping with your head raised the first night helps, and so does keeping the lips moist with Vaseline or Aquaphor.
“I always tell patients not to judge their lips the first morning,” Dr. Naderi says. “No face is perfectly symmetric, and swelling is rarely even from side to side. Give it time, and let your lips settle before you decide how you feel about the shape.”

What should I do in the first 48 hours?
For the first two days, we suggest keeping things easy. Please avoid strenuous exercise for 48 hours, keep the lips moisturized, and continue gentle icing when it feels good. Please do not massage or press on your lips unless your physician asks you to. For any soreness, Tylenol is the better choice; ibuprofen can add to swelling and bruising.
Everyday life can carry on beautifully. Lipstick is fine within a few hours, and a lip balm helps for a day or two because the lips can feel dry. Kissing is fine the same day, a little alcohol is fine, and flying or dental work is fine from the next day. Our full Botox and filler aftercare instructions cover the rest.

Is it normal for my lips to look uneven or lumpy while they heal?
Yes. In the first several days, swelling, tenderness and some firmness are all expected, and lips often look uneven while they are swollen. Filler also feels a little firmer than your own tissue at first. As the swelling eases each day, the lips soften and the shape you saw in the mirror on treatment day returns.
“A lip that looks a touch uneven on day two is almost always swelling,” Dr. Naderi explains. “I’d much rather a patient wait out those first two weeks than press on the lips trying to fix something that will settle on its own. If anything still concerns you at two weeks, we would be delighted to see you.”
Bruising is the other common visitor, because the lips bruise more easily than most areas. A small bruise near the lip border fades over one to two weeks. A slightly darker lipstick shade is a lovely way to hide it in the meantime, and arnica may help a bruise fade a little faster.

What do my lips look like once the swelling settles?
Once the swelling settles, the lips are softer and look much more like your own lips on a good day. Most patients feel comfortable in photographs again, whether the occasion is a family gathering in Potomac or a dinner out in Bethesda.
If you would like a little more volume, the best time to add it is once your result has fully settled. “You can’t make thin lips go full in one shot,” Dr. Naderi says. “That’s just not going to happen. We build gradually, and each visit gets its own time to settle.”
For very thin lips, that gradual approach means filling in stages, a few weeks apart, so the lips stretch gently and stay soft. Each stage has its own short swelling cycle, and you will know exactly what to expect after the first one. Our lip type guide shows how we usually approach different lips, from a thin upper lip to lips that have thinned with age.
Is my swelling normal, or should I call?
Almost every concern in the first week turns out to be ordinary healing. Rarely, filler can block a blood vessel. The signs are different from normal swelling: severe pain, pain that keeps getting worse, skin that turns white, gray, blue or blotchy, or any change in your vision. If you notice any of these, please call us right away; your physician can dissolve the filler immediately with hyaluronidase.

“Knowing the anatomy of the lips and injecting slowly lowers this risk considerably,” says Dr. Naderi, a former national clinical trainer for Allergan and Medicis who taught other physicians how to inject filler. “We would always rather hear from you early. A quick call brings peace of mind, and we are glad to take it.”
Can I help my lips swell and bruise less?
A little preparation goes a long way. For two weeks before your visit, we suggest avoiding nuts, seeds, fish and fish oil, aspirin, ibuprofen, naproxen and supplements that thin the blood, such as vitamin E. If you take a prescription blood thinner, please ask the doctor who prescribed it, and never stop a medicine you need for a cosmetic treatment. If you get cold sores, tell us ahead of time so your physician can prescribe an antiviral to start before your injections.
On the day of your visit, please eat and drink normally and come without lipstick, so your physician can study the natural border and shape of your lips before placing any filler. If you are joining us from Arlington or Alexandria on a workday, leaving a little extra room in your schedule lets you arrive early and relaxed.
Your physician chooses the filler for your lips and your goal, whether that is Restylane Kysse, Juvederm Volbella XC, Juvederm Ultra XC, RHA Redensity or RHA 2, and many patients do well with more than one. A filler that holds its shape is often used to define the border, and a softer filler for fullness or fine lines. If you are drawn to a particular look, our lip shape guide is a lovely place to start the conversation.
When should I book lip filler before a holiday or event?
We suggest planning lip filler at least two weeks before any important event, so swelling and any bruise have time to settle. For Thanksgiving on November 26, that means booking by November 12. For Christmas, plan by December 11, and for New Year’s Eve, by December 17. If you have never had lip filler, a first treatment a month or more ahead leaves room for a touch-up, so aim for November 25 for Christmas and December 1 for New Year’s Eve.

A little thought about the week itself helps too. If your weekdays are full, from a downtown Washington DC office to a busy family calendar in Vienna or Great Falls, a Thursday or Friday visit lets the fullest morning fall on a quieter weekend at home. A Monday visit suits those who would rather have their lips settled by a Saturday event.

Lip filler swelling: frequently asked questions
How much does lip filler cost at The Naderi Center?
Lip filler is $900 per syringe, whichever filler your physician chooses: Restylane Kysse, RHA Redensity, RHA 2, Juvederm Ultra XC or Juvederm Volbella XC. We no longer offer half syringes, and most first-time patients need one syringe.
Do lip injections hurt?
Most patients find them very tolerable. Numbing cream is applied for 20 to 30 minutes, the lip fillers contain lidocaine, and the injections themselves take about 5 minutes. We rarely need a dental block.
How often will I need a touch-up?
Most patients return every 6 to 12 months. A first treatment can fade a little sooner, and with regular touch-ups many patients find they need less filler to keep their result.
Can I have lip filler while pregnant or breastfeeding?
We kindly ask patients to wait until they are no longer pregnant or breastfeeding. We also wait if there is an active cold sore or infection around the mouth.
Can you correct lip filler I had done somewhere else?
Often, yes. Lips that look overfilled, lumpy or uneven can usually be dissolved with hyaluronidase, which is $1,500, and then rebuilt with a better shape at our usual lip filler price. Learn more about correcting filler from another office.
