Brow Lift in the Washington DC Area: Reston, VA and Chevy Chase, MD

Dr. Shervin Naderi, a double board-certified facial plastic surgeon, performs brow lift surgery for patients of The Naderi Center in Reston, Virginia and Chevy Chase, Maryland. A brow lift raises low, heavy brows back to a natural position, which takes weight off the upper eyelids and the tired, worried look off your face. Dr. Naderi’s main technique is an endo-style brow lift through small incisions hidden behind the hairline.

Every brow lift is done in the operating room at Prince William Surgery Center under general anesthesia, and most patients are back at work in 1 to 2 weeks. Patients come to us from across Northern Virginia, Washington DC and Maryland, including McLean, Great Falls, Vienna, Arlington, Bethesda and Potomac.

Your Brow Lift Surgeon: Dr. Shervin Naderi

Dr. Shervin Naderi, MD, FACS, is a double board-certified facial plastic surgeon and the founder of The Naderi Center in Reston, Virginia and Chevy Chase, Maryland, with more than 20 years in practice and more than 100,000 procedures performed. He operates only on the face and neck, and his goal is brows that look rested and natural, never surprised or “done.”

Honest advice: Dr. Naderi also performs his own injectable treatments, so his advice isn’t skewed toward surgery. If Botox will give you what you want, he’ll tell you. If your brows are too low for Botox to help, he’ll tell you that too.

What Can a Brow Lift Fix?

A brow lift corrects brows that have dropped with age, which can make you look tired, sad or angry when you aren’t. The goal is a rested, natural brow, never a surprised one.

Why the Brow Comes First

Many people who come in asking for eyelid surgery actually have a brow problem. Dr. Naderi looks at your brows, upper eyelids, forehead and the muscles that move them together, because they work as one unit.

The Brow Is Often the Real Problem

In Dr. Naderi’s experience, about 9 out of 10 older patients who come in for upper eyelid surgery also need their brow reset.

What feels like extra eyelid skin is very often a brow that has dropped.

Skin Removal Alone Can Backfire

If the brow is low and only eyelid skin is removed, the brow can be pulled down even further. The eyes can look worse, and the hooding at the outer corners stays.

Upper eyelid surgery is a beautiful procedure, but only when the brow is in the right place.

Heavy After Botox?

The muscle that creates forehead lines is the same muscle that lifts your brows. If your brows are already low, Botox that smooths the whole forehead can leave them feeling very heavy and droopy.

For these patients, the answer is often a brow lift, not more Botox.

Your Consultation

What Dr. Naderi Checks Before Recommending Surgery

Before recommending a brow lift, eyelid surgery or both, Dr. Naderi looks at four things together, because they all go hand in hand.

  1. Your Brow Position

Most of the time, though not always, what feels like extra upper eyelid skin is really a brow that needs to be repositioned.

  2. Your Upper Eyelid Position

Some patients have ptosis, where the upper eyelid itself sits lower. That’s different from extra skin, and it must be corrected as well.

  3. Your Forehead Height

If your forehead is really long, Dr. Naderi also removes a strip of skin at the hairline when he lifts your brow.

  4. Your Forehead Muscles

The frontalis muscle that creates your forehead lines is also the muscle that lifts your eyebrows.

A Real Second Opinion

A patient Dr. Naderi had known for many years came in after two eye specialists recommended removing her extra eyelid skin. He told her that removing the skin alone would be a mistake. Her brow, her ptosis and her extra skin all needed to be corrected.

“All three have to be done at the exact same time.”
Dr. Shervin Naderi

Dr. Naderi’s Endo-Style Brow Lift

Dr. Naderi’s main brow lift technique is what he calls an endo-style brow lift: endoscope-style surgery, done without the endoscope. It is one of the techniques that has evolved over his more than 20 years in practice.

  • Small incisions hidden behind the hairline, so the scars are out of sight.
  • The brow is lifted and secured in its new position. When Dr. Naderi uses Endotines, these small dissolvable anchors hold the brow while you heal.
  • A natural brow position, never a surprised look. For men, he keeps the brow flatter, without a feminine arch.
  • Often combined with upper eyelid surgery, so the brow and lids are corrected together in one surgery.

Surgery is done under general anesthesia in the operating room at Prince William Surgery Center.

Dr. Naderi’s Approach

What Makes Dr. Naderi’s Brow Lift Different

Brow lifts used to scare people. Until just a few years ago, many brow lifts left patients looking surprised, like a deer in the headlights. Dr. Naderi does it very differently.

  1. Released, Not Pulled

Dr. Naderi uses a deep plane, endo-style dissection to lift and reset the brow. There is no yanking or tugging. He releases everything and lets it settle in the right place.

  2. Reset to Where It Used to Be

The goal is to reset your brow to where it used to be, not lift it to an unnatural height. Sometimes the brow doesn’t even come up that much; the lift is about taking the tension off.

  3. Incisions Planned for Your Forehead

The incisions are very small, at the hairline or slightly behind it. If your forehead is really long, a strip of skin is also removed at the hairline (a pretrichial lift). For men with receding hairlines and deep lines in the middle of the forehead, one of those lines can be used for a mid-forehead or even a direct brow lift.

  4. Completely Asleep, With an Option for Less Bruising

You are completely asleep under general anesthesia, because the entire scalp is released. If you can’t tolerate bruising, Dr. Naderi can place a drain, which really minimizes it and comes out the next morning.

“We’re not trying to yank the brow up. There’s no real pulling.”

Dr. Shervin Naderi

“We are trying to basically reset the brow to where it used to be. Not lift it to a crazy position.”

Dr. Shervin Naderi

“The goals are to look rested, refreshed, never surprised.”

Are You a Good Candidate for a Brow Lift?

Good candidates are healthy adults whose brows have dropped and make them look tired, sad or older than they feel. At your consultation, Dr. Naderi examines your brows, eyelids and forehead together to decide what will actually help.

Is It Your Brows or Your Eyelids?

In Dr. Naderi’s experience, about 9 out of 10 older patients who come in for upper eyelid surgery also need their brow reset. What feels like extra eyelid skin is very often a brow that has dropped.

A brow lift may help if

Signs your brows have dropped

  • Your brows sit lower than they used to
  • Skin hoods the outer corners of your eyes
  • You raise your brows all day to keep your eyes open
  • Your forehead feels heavy after Botox
  • One brow sits noticeably lower than the other

Eyelid surgery may help if

Signs of extra eyelid skin

  • Extra skin folds over your eyelid crease
  • Eyeshadow disappears into the fold or smudges
  • Extra skin is still there once your brows are in the right place
  • You’d like your brows and eyelids done in one surgery

Why it matters: removing eyelid skin alone can pull a low brow down even further. Dr. Naderi looks at your brows, eyelids and forehead together.

Who Should Wait on a Brow Lift

A brow lift is elective, so the timing should be right. Dr. Naderi may recommend waiting if you:

  • Are pregnant or breastfeeding.
  • Have a medical condition that isn’t under control yet. It should be well managed before any elective surgery.
  • Smoke or use nicotine. All nicotine, including vaping, patches and gum, has to stop at least two weeks before surgery.
  • Can’t stop aspirin, ibuprofen or blood-thinning supplements for the two weeks before surgery. If you take a prescription blood thinner, check with your doctor first.
  • Have a flight or big event coming up soon. You shouldn’t fly for 7 to 10 days after surgery, and flying can add swelling for the first few months.
  • Want a different look, not a refreshed one. The goal is brows that look rested and natural, never surprised.

Finding the Right Brow Position

Not Too Low. Not Too High. Just Right.

Too LowHeavy and HoodedWhen the brow comes down, the eyelid skin bunches up into a hood, and you can look tired when you aren’t.
Too HighSurprisedThe old-style brow lift look: pulled too high, like a deer in the headlights.
Just RightRested and RefreshedDr. Naderi’s goal: reset the brow to where it used to be. Rested, refreshed, never surprised.

Illustration for education. Your plan depends on your own brows, eyelids and forehead.

Brow Lift or Eyelid Surgery?

Heaviness above the eyes can come from a low brow, extra eyelid skin, or both. Removing eyelid skin alone won’t fix a low brow, and it can pull the brow down further. Dr. Naderi checks your brow position at your consultation, and many patients do best with a brow lift and upper eyelid surgery together. To learn more, read Should You Get a Brow Lift or a Blepharoplasty?

Brow Lift or Botox?

Botox can soften forehead lines, but it can’t lift brows that have truly dropped. The muscle that creates forehead lines is the same one that holds your brows up, so if your brows are already low, smoothing the forehead with Botox can make it feel very heavy. For these patients, a brow lift is the real fix.

Brow Lift for Men

Brow and eyelid surgery are different for men and women. For men, the goal is a natural, masculine brow that looks rested, not lifted.

A Flatter Brow

When a man’s brow needs to be reset, Dr. Naderi avoids creating too much of an arch, so the brow never looks feminized.

Thinning Hair or a Receding Hairline

Incision placement matters more when hair is thin. Dr. Naderi can use specialized incision techniques to keep scars discreet. For some balding men with deep forehead lines, the lift can be done through an incision hidden in one of those lines.

Brows and Eyelids Together

Many men also benefit from conservative upper eyelid surgery, with a lower, flatter crease and only the skin that needs to go removed.

Is it the eyes? Is it the brows? Which one is it? Stay tuned to find out.

A lot of patients, it’s a very common problem. They come in here asking for excess eyelid skin removal, droopy upper eyelids, eyelid hooding, etc. Nine out of 10 of them over a certain age, it’s also partially due to the brows. When the brows descend, when the brows come down, the eyelids tend to bunch up. I’ve had this issue since I was in my 20s. It’s genetic. Some people develop it later in life. Some people, their eyebrows are so high that they never get that issue. But those need to be examined.

If your eyebrows are lower and you go ahead and remove extra eyelid skin, you’re going to make the problem worse. You’re going to pull the eyebrow down even further. In fact, you will not be able to clean up this lateral hooding without getting the brow into a better position.

Now, brow lifts used to scare people. In the old days, and I’m talking about just a few years ago, surgeons would do this. This is so simple. This part of brow lift is very easy. So everybody looked like a surprised deer in the headlights. Terrible.

Now we do it very differently. A deep plane dissection, endo style, allows us to lift and reset the brow. We’re not trying to yank the brow up. There’s no real pulling. We’re not trying to tug it up. We’re releasing everything and letting things settle in the right place.

So when I do an endo-style brow lift, the incisions are very small, at the hairline or slightly behind it. Unless the forehead is really long, and then I have to remove a strip of skin as well, called the pretrichial. But the brow must be examined.

Now, it gets tricky in men with receding hairlines. But if they have really deep lines in the middle of their forehead, developed from not doing Botox, then that gives us an opportunity to use that area for a mid-forehead or potentially even a direct brow lift.

But the brow must be examined with the upper eyelids. If you just go and get an upper blepharoplasty and don’t address the brow, you may actually end up with a worse situation and a sad puppy look, where this kind of gets pulled down and it’s hooded. It’s terrible.

Let me mention something else. If you go to a med spa or a doctor’s office, even here, and they try to talk you into, let’s put a lot of Botox here, let’s smooth out your forehead, your brow problem will get worse. So patients go and get that done, and then they go and tell all their friends and family, Botox is bad, don’t do Botox. Botox is not bad. Botox is great. You just had bad injection decisions and a bad pattern.

This muscle that creates the lines, called the frontalis, also lifts the eyebrows. If you don’t want to have lines, then we have to put Botox there. Guess what happens? Botox to the muscle that lifts the eyebrows means the eyebrows aren’t getting lifted. So if you already have heavy, hooded upper eyelids and a droopier brow position, and you Botox the heck out of your forehead, the problem’s going to get worse. So again, Botox is not the problem. The injector is the problem.

A brow lift is about an hour, hour and a half procedure. You’re completely asleep, very comfortable. There’s no pain whatsoever during surgery. I do it under full anesthesia, so you’re super comfortable, because I have to release the entire scalp.

You go home. Small incisions are sutured, sometimes stapled, but most of the time I suture them. If it’s way back in the hairline, staples are fine. But if it’s closer, I will stitch them. The sutures are dissolvable. They fall apart by themselves. Sometimes I’ll place a drain if somebody says, “I can’t tolerate bruising.” If you don’t mind bruising for a few days, we don’t need a drain. But putting a drain means that you really minimize the risk of bruising. The drain comes out the next morning.

It’s a straightforward surgery, straightforward recovery, and the results are also straightforward. We are trying to basically reset the brow to where it used to be. Not lift it to a crazy position. Reset it to where it is. Sometimes it doesn’t even come up that much. It’s just a resetting and taking the tension off of it. That’s the beauty of a natural, deep plane approach to the brows, an endo-style, endoscopic-style lift. That’s a brow lift, and it’s important.

So again, the goals are to look rested, refreshed, never surprised. That’s terrible. Don’t go to somebody that does that. If you want to come see me, I have an office in Chevy Chase, Maryland, right on the border of Washington, DC and Chevy Chase, Maryland. And the other office is in Reston, Virginia, at the Reston Hospital campus.

Transcript lightly edited for readability. About your stitches: they are usually dissolvable, and about 1 week after surgery Dr. Naderi removes some of them to help the rest dissolve faster.

What to Expect on Surgery Day

A brow lift can be done on its own or together with upper eyelid surgery, a facelift or other procedures in the same surgery. Here is what happens, from two weeks before surgery to the ride home.

  1. Getting Ready, Starting 2 Weeks Before

    • Your prescriptions are sent to your pharmacy at least two weeks ahead. Open the bag early and make sure nothing is missing.
    • Start Dr. Naderi’s anti-bleeding, anti-bruising diet. Stop aspirin, ibuprofen, naproxen, alcohol and blood-thinning supplements.
    • Stop all nicotine at least two weeks before surgery. Four weeks is better.
    • Starting a week before, spray hypochlorous acid on your forehead, hairline and scalp three times a day.
    • Take two showers, the night before and the morning of surgery, shampooing twice each time.
    • Nothing to eat or drink after midnight the night before.

Read the full brow lift instructions

  2. In the Operating Room

Dr. Naderi performs every brow lift under general anesthesia in the operating room at Prince William Surgery Center, where he is medical director. Anesthesia is given by board-certified anesthesiologists and the nurse anesthetists (CRNAs) they supervise. Surgery takes about an hour and a half.

    • Endo-style brow lift: through small incisions hidden behind the hairline, Dr. Naderi lifts the brow and secures it in its new position, sometimes with Endotines, small anchors that dissolve on their own.
    • Temporal or direct brow lift: when only the outer brow needs lifting, or when another approach fits you better.
    • Combined procedures: upper eyelid surgery is often done in the same surgery.
  3. Going Home

You go home the same day with a pressure wrap. A responsible adult needs to drive you and stay with you at first; you can’t be released to an Uber or taxi. The day after surgery, Dr. Naderi checks in with you by telehealth and tells you when to take off the pressure wrap. If you have a drain, you’re seen in the office to have the drain and wrap removed. Drains help patients heal faster, with less bruising and swelling.

Brow Lift FAQs

At The Naderi Center, Dr. Naderi’s surgical fee for an endo-style brow lift is $9,000 to $15,000+. Anesthesia and surgery center fees are separate and depend on how long the surgery takes. You’ll get an exact quote at your consultation, and Cherry financing is available.

No. A brow lift at The Naderi Center is cosmetic, so it is self-pay and is not billed to insurance. Cherry financing is available if you would like to pay over time.

It is Dr. Naderi’s main brow lift technique: endoscope-style surgery through small incisions hidden behind the hairline, done without the endoscope itself. The brow is lifted and secured in a natural position, sometimes with Endotines, small anchors that dissolve on their own.

Yes. Both names describe the same surgery, which lifts low brows and the forehead above them.

About an hour and a half. Every brow lift is done under general anesthesia in the operating room at Prince William Surgery Center, and you go home the same day.

Most patients are back at work in 1 to 2 weeks. Don’t fly for 7 to 10 days. No exercise or lifting over 20 pounds for the first month, then build back slowly toward full activity by about 3 months.

Your result should improve month after month. Don’t expect to love it at one month; it usually takes about 3 months to feel comfortable with your result. Numbness and itching of the scalp can take up to a year to settle.

A brow lift doesn’t stop aging. It resets your brow into a better position, and the aging process continues from there.

Usually only some of them. Your stitches are usually dissolvable, and about 1 week after surgery Dr. Naderi removes some of them to help the rest dissolve faster. If you also have staples, they come out at about 10 days.

Some patients do. If you have a drain, you’re seen in the office to have the drain and pressure wrap removed. Drains help patients heal faster, with less bruising and swelling. Without a drain, Dr. Naderi checks in with you by telehealth the day after surgery and tells you when to take off the wrap.

The day after surgery, Dr. Naderi checks in with you by telehealth, or sees you in the office if you have a drain. About 1 week after surgery, Dr. Naderi removes some of your stitches to help the rest dissolve faster.

Most patients do well with their prescription pain medicine, or Tylenol if the pain is mild. Headaches, migraines or nausea are common the first couple of nights. Numbness and itching of the scalp are expected and can last for months.

Dr. Naderi prefers that you are asleep, so you stay still and comfortable and he can work under the best operating conditions. Many brow lift patients also have upper eyelid surgery or another procedure at the same time. Anesthesia is given by board-certified anesthesiologists and the nurse anesthetists (CRNAs) they supervise.

Dr. Naderi uses a variety of incision techniques, chosen for your forehead shape, your hairline and the part of the brow that needs to be lifted. With the endo-style and temporal techniques, the small incisions are hidden behind the hairline. Incisions can look more noticeable at first and usually improve with time. A direct brow lift leaves a scar just above the brow, so it is used mainly when thick brow hair helps hide it.

No. Dr. Naderi takes great care to keep your brows in a natural position and in balance with the rest of your face, so you look rested, never surprised or overdone.

Yes, and many patients do. In Dr. Naderi’s experience, about 9 out of 10 older patients who come in for upper eyelid surgery also need their brow reset. Doing both in one surgery means one anesthesia and one recovery. See eyelid surgery.

The muscle that creates forehead lines is the same muscle that lifts your brows. When Botox smooths the whole forehead, that muscle can’t hold the brows up, so they can drop and feel heavy. If your brows are already low, a brow lift may be the better answer. Watch Dr. Naderi explain.

Botox can give a small lift in some patients, but only while the muscle that lifts the brows is still working. It can’t raise brows that have truly dropped, and for those patients, forehead Botox can make the brows feel heavier. Dr. Naderi will tell you honestly which one you need.

Endotines are small dissolvable anchors that hold the brow in its new position while you heal. You may feel them under the skin, and they can be tender for about 6 months, until they dissolve.

Yes. Dr. Naderi plans the incisions around your hairline. For a high forehead, the lift can be done along the hairline, which can also shorten the forehead. For thinning hair, he can use specialized incision techniques that keep scars discreet.

You may shower and shampoo 48 hours after surgery, or 48 hours after a drain comes out. Let the water and shampoo run over your scalp, and don’t rub or massage it. Skip hair dryers while your scalp is numb.

Every brow lift is performed by Dr. Shervin Naderi, MD, FACS, a double board-certified facial plastic surgeon with more than 20 years in practice, at Prince William Surgery Center, where he has been medical director since 2023.

He operates only on the face and neck, and he looks at your brows and eyelids together instead of treating them one at a time. His endo-style technique has evolved over more than 20 years in practice. He also performs his own injectables, so his advice isn’t skewed toward surgery.

Upper eyelid surgery is the most common pairing. A brow lift can also be done in the same surgery as a facelift, neck lift or laser resurfacing.
