What Happens During Buccal Fat Removal?

Dr. Naderi removes the buccal fat pad through an incision inside the mouth near the bite line, with the opening of Stensen's duct, where saliva comes out of the parotid gland, kept under the protection of the retractor. The fat pad is freed with gentle teasing, with no yanking or pulling, and he says it is about a half-hour procedure with a quick recovery.

This video shows surgery.

So we're doing a Bichat's fat pad, a buccal fat pad removal. Come on in here. You'll be able to see it really well if you zoom in there. So up here, it's the opening of Stensen's duct. Sometimes you can actually see saliva come out of it. It's under the protection of the retractor. We've got the bite line right here, and this is Bichat's fat pad. I'm going to show you both sides. It's pretty large. On men, you can definitely resect a little bit more, get them really nice and contoured.

And it's out. We're going to just basically go ahead and free it up. It's just simple, gentle teasing, and it comes right out. There's no yanking or pulling. Very simple maneuvers, and you grab it, and then you just cauterize it. I'm going to compare the two sides. And if you actually go right here, see, very gentle teasing, very gentle teasing, make sure there's nothing in there. And then we're going to cauterize and take it out, and then you can compare it to the other side. You can actually compare the two sides in a second.

All right, so we're doing a buccal fat pad excision. Here we have the bite line, which is basically between the two teeth. Above us, right there, that's Stensen's duct. That's where the saliva comes out of the parotid gland. And the incision is right here, and the buccal fat pad has been delivered slowly, carefully, gently, and we're going to excise it. And by removing this, this young man is going to get a little bit more contoured cheek.

So that's basically the buccal fat pad right here, the Bichat's fat pad. Okay, so here are the two pads. If you focus, you'll see these are the pads here. So this one was sitting right here. You can come over here and you kind of see where that one, right there, that's where that one was sitting. And then this one was sitting right there. So by taking these out, the cheeks are much more developed, and he's going to have more contour there. So that's quick and easy, less than an hour. It's about a half an hour procedure to do. Quick recovery.
