Question: Hi Dr. Naderi, I am considering a lower lid blepharoplasty to remove under eye bags. I am 44 years old. Do I have to worry about my eyes looking hollow after the procedure? Also, what type of anesthesia is used and is the procedure performed in your office or a hospital? Approximately how many lower lid bleph procedures have you performed? Hi, As with any cosmetic surgery, the lower lid blepharoplasty can be done properly or poorly! If done poorly or over aggressively then lower lid hollowing can be one of the complications. The key in eyelid surgery is not to remove too much fat or every single extra pinch of skin. If too much fat is removed then the eyes may become hollow and dark and sunken in. If too much skin is removed then lid retraction or ectropion or inability to close the eyes or dry eyes can occur. The surgery for the lower lids can be done "external" or "transconjunctival." I typically perform these surgeries at the Hospital but occasionally I may make exceptions and perform them in the office - occasionally! Anesthesia for such cases are usually local or twilight but general is an option. I have probably performed hundreds of eyelid surgeries over the last 9 years but I do not know the exact number.
