Dear Dr,

I am having the same problems as Trish and my Dr keeps saying not to worry, it will dissolve on it's own soon but after reading your answer to Trish in regards to the filler forming scar tissue, I am worried if it is going to go away. My Dr is not sure which stroid injection to use and which steroid will dissolve or remove the filler as there isn't much informationavailable. What will you recommend?

Hi,

Unfortunately I have to tell you the same thing I told Tris which is we dont have much experience with novielle gel plus in the US.  Its not a common filler and I have never injected it.  Regretfully, I don't have an answer for you other than the fact that you should go see a few other plastic surgeons or dermatologists and see what they have to offer.  I would continue calling the company in the hope of getting an answer or solution from them.

My english is not good and maybe I don't explain to you good about to come back to my pre-op .....

I Know it is almost impossible to return perfectly how I was before, but I would like to Know If I can have a normal chin again with my S form again...the doctors here told me than I can have a normal chin with steroid or with surgery taking out the scar tissue...( the  more prominent part is right under my  bottom lip I suppose it  is because of the scar inside my  mouth) this part is a bump it is soft but it doesn't shink.
I know you are an expert in faces and I know you are a very good doctor for this reason i would like your opinion too.

I don't want live with this for the rest of my life.

About your questions:

The doctor in my country put a implant .But 18 days later he cut the implant and put again (because he told me that it was to big for me) and then 15 days later it was too inflamated and I needed  to come back to Atlanta and I ask him to remove it and he did it.
Then 3 days later I had a lot fluid inside and he take out the liquid with a needle . And was all good but a week later , a hard ball form in the chin.
He told me that the ball would go away  on its own in 3 months. But  the ball improved  a little but not so much....the inflamation is a lot better but now the problem is the scar tissue.

  I think that if the part under my lip improves, my chin will look almost normal.

Hi Maria,

Sorry to hear all that you have gone through.  A good plastic surgeon should know ahead of time what size implant will fit best on his or her patient.  It does sound like too much was done to your chin too quickly and that will create lots of scar tissue.  In such cases, you have to be under the care and supervision of a good plastic surgeon to make sure things are not infected and that the area is stable. 

Allowing time to pass and using steroid injections to soften the scar tissue is a good thing usuallly before rushing back into surgery again.  Once time has passed (at least 12 months from the last surgery) and enough steroids have been used, surgical scar removal may be a good option.  Although you will not get back your exact old chin, your surgeons should be able to improve the chin significantly. 

See what Dr. Silver recommends. I wish you luck.

Hello Doctor
I"m the mother of 38 years old who had a implant in my chin in Ecuador
and the doctor remove 18 days later and then remove again 15 days later....
Then I have a golf ball in my chin ( now is 9 months since the last remove) the deformity of my chin is more rigth under my lip for this reason i don't have the "s" form in my chin.

I saw doctor Tolson first and he told me that the problem is inflamacion and scar tissue then I went to a second opinion and Doctor Silver think that the roblem is scar tissue .....and both of them think about the steroid can help....

The two Doctors in the past work toguether so they know each other and Dr Tolson is closer to my home and he will help me with the inyeccions of steroid...

The Doctor inyect me a steroid in November and two weeks later he didn't see any diference and he inyect me again and the second time the part under my lip beginning to soft ..but no more changes. the doctor wait until the last week and he put me a inyeccion again but I still don't see any diference.


The implant was intraoral.

I went to the doctor in my country for otoplastic and he insist that i need a chin surgery too and neck lipo....and he told me If you don't like the chin implant I can remove and you return to your normal chin... this was my first surgery and I believed in him.

The questions are:
1.- Do you think the steroids help me to returnd to my pre-op chin?

2.- Do you think the results will be uniform or with deeps ?

3.- Do you think I can come back to my normal chin and how long  this problems can be resolve with steroids

Hi Maria,

Now I do remember seeing your question and I think I sent you an email in response but here it is again:

Without seeing you in person and examining you, I cannot tell you what YOU should do.  You have seen Dr. Silver and he is a great Facial Plastic Surgeon who is well known for cosmetic plastic surgery as well as head and neck surgery and reconstructive plastic surgery. 

I have some questions for you now:

1) I am not sure what you mean by the implant was removed 18 days later and then again 15 days later?!? 
2) Was it trimmed at 18 days instead of complete removal?  Or did he remove the implant and put in a different one which he also later took out?
3) Why was it removed in the first place and the 2nd place? It just seems so hasty and rushed without letting time for post-op swelling to go down before making a proper decision

The fact is that it has been 9 months now so whatever is left is mostly scar tissue as long as the entire implant has been removed before.

It is almost impossible to get back your old chin.  That is not realistic.  The goal should be to improve your current situation.  Again, I cannot tell you what to do without seeing you and examining you but make sure you see some other good surgeons if you are not sure yet and do not rush into any other procedure. 

Conservative steroid injections are fine as long as done carefully and should relax some of the scar tissue.

Question:

Following the advice of my plastic surgeon I received fat transplant to the upper eyelids in the form of injections. The results were disastrous; the right eye especially has calcified lines of fat or scar tissue and the inner upper corner an especially noticeable nodule. My question is have you ever successfully removed injected fat from the upper eyelids? If not, is there someone in your circle of professionals experienced with this type of problem? Thank you in advance.

Hi,

I have seen plenty of poorly done fat injection and around the eyes and lips, nodules can become a large problem.  Other areas are more forgiving. 

I would have to see you to determine what is the best approach.  Most of the time the area can be improved greatly with direct excision of the nodules or hypertorphic deep scars.
