Hi Dr.

My name is Reddy I am 31 years old I have a burn scars mark under my chin towards neck which is looking bad every ever I go people ask insult and don't talk with me..I feel embrass.It been years I Quit going to family parties. Is there any procedure ? please help me ?

Answer:

Dear Reddy,

The treatment depends on the locatio, size, nature and color of the scar and the surrounding tissue.  Excision and repair may be needed.  Do you have a picture you can email me?

Question:

I was in a car accident at the age of 4. I had 7 fractures to my skull along with many other  injuries. I was very lucky to survive and I was the only survivor. My scars on my forehead are shaped lhe letter y and the bottom goes down across my eyebrow into the top of my eyebrow. Where the scar meets my eyebrow is extremely itchy, I itch it all the time. My eyebrow bone is also different and more protruding. The scar is raised, much wider and very red at the eyebrow ad halfway into my forehead. I have another scar going acrossed my cheek that is more indented. When I was 19 I saw a doctor in NYC that did a lazer on my forehead that burned the scar, It is actually more noticeable and much redder and definitly itchier than ever. He also did a lazer on my cheek that took the red pigment out and left tiny white circles along the scar. I am now 29 and have worn long bangs my whole life. I know I would have been pretty if only I didn't have these scars on my face and they are a everyday reminder of the tragic accident. I think after I had the procedures at age 19 I just thought there was no hope. Have there been advances in this type of scar revisions? If I did have options what would they be? Also, how much do these type of procedures cost?

You have obviously been through a lot and going through more procedures should be only done if the procedures are going to actually improve things.

I am looking for a surgeon who can do Scar Revision on my sister's arm. She is 27, and she has a very bad, deep and old scar from burning when she was only 3. She never wears short sleeve clothes, even in hot summer days. And I've always wondered how much it costed. She is in Mongolia right now. If we send you her scar pictures would you give us an estimate on cost. She can come to USA during her vacation, if you think that is neccessary. Please give us some information. Dear Mugii,

Thanks for the email.  A plastic surgeon should be able to give you a rough estimate on cost based on good pictures most of the time.  I do not do any body plastic surgery and my practice is focused on facial plastic surgery and especially Rhinoplasty.  If you are looking for doctors in the DC area, email me at info@nadericenter.com and I will give you some names.

Samantha, a friend of mine reffered me to you since she said she will be your patient soon.  I have scars on my head and also on both end of my eyes, is there anyway that you can help me to remove it? I also have large pore on my face and I just wonder if you have any treatment for it? I also need to have a rhinoplasty done, and perhaps a mid face lift.  Do you have a package discount for patient who has a lots of surgery done at the sametime? it would be very helpful if you do.  IThanks so much for yourDr. Naderi!

Hi,

I would be happy to see you. 

Scars cannot be erased but many times they can be improved.  It just depends.  Large pores cannot be removed but with a good skin care regimen, my Master Aestheticians can help you reduce the appearance of your large pores.  We certainly can talk about your nose and face.  We do take into consideration when patients require multiple procedures but before we get so far ahead, we need to meet so we can talk and I can examine you and show you what is realistically possible.

I have been searching on the internet for a solution to my acne scarring and I came across your website. I think it is absoletely wonderful, that you answer peoples questions, in such a kind and caring manner. I have deep ice pick type scaring, on my face and it has affected all areas of my life and I would love to do something about them. Do you think you can help? If so, how hard is it to get an appointment with you. I live about 3 hours away. Thank you for any help that you can provide.

Hi Dr. Naderi,

I have a depressed scar on my cheek similar to the woman on your photos page.  You mention she underwent a 2 part excision.  What does that mean?  Also,  what size scar was she left with after she healed from the surgery.  I have tried filler and total fx with little success and am now seriously considering excision, as I really would like to improve this scar.  Is there anything that should be done that would lead to the best possible outcome.  Also, what is the risk of making the scar even worse. Thanks for your time.

Answer:

Hi Paula,

A two part excision is also known as serial excision.  This can be done for large moles or large scars or small areas of hair loss on the scalp.  When one excision would result in a very large excision and a long scar, the surgery is split up and part of the scar (or mole or alopecia) is removed then the rest is removed after the area has healed.  It can even be done over 3 to 4 times. 

The length of scars is much less important than their orientation, color, surface characteristics and location. 

Depressed scars dont respond to Lasers alone.  Sometimes filling with Restylane or Juvederm does not even work if the scar is tethered down.  The filler just ends up to the left or right of the scar.

Releasing the scar and filling it is an option.  Excision and dermabrasion is an option.  There are multiple options in scar revision but the options are limited by the patient's skin color and ethnicity.

I'm Maria again ..i couldn't send you my pictures but these weekend my husband will help me.

I went to Dr.Silver again and he still think that I have a scar tissue in my chin and inyected  me  more steroid but this time stronger 30 mg because the last time didn't work ....
He inyected me inside my mouth directly  in the scar tissue...I think was a good idea.

I hope this time work ..because I need a normal chin again with the 'S: form under my lips...

I forgot to ask him in how many days i will see some diference?

Hello Maria,

It may take days to weeks to see any results from the steroid injection into your chin scar.  Make sure you follow up with him as he is your plastic surgeon now.

Yes I already saw Dr Silver and he told me that is good idea to put steroid in the scar tissue in the mentolabial sulcus to help shrink the tissue....I hope this helps me, because if this part goes down and I have the "S" form , the chin will look good, and can be easier for me to wait more time until see if all scar tissue improve.

These are the last questions :

-I don't Know if the steroid work enough to have a normal "S" Form because the skin in the mentolabial sulcus is normaly very thin.
Can the steroid relax the scar tissue until it is thin again?

-In case the steroid is not working,is the operation to remove the scar tissue is easy and successful?

Hi Maria,,

You're welcome.

Steroid injection is usually done multiple times.  Some patients need 2-3 injections and some need 10-15 injections.  There are also different strengths of steroids from K10 to K40 (40 is stronger). 

The ONLY way to tell if the steroid injections will work is to do them.  There is no guarantee that it works but usually it helps. 

Removal of the scar tissue should not be that difficult around the chin. 

Your best bet is to stay under the care of a good plastic surgeon and take it a day at a time.  If there is no nerve damage or infection then the scar tissue can be reduced relatively easily and your chin should improve.  Again, getting your "old chin" back is impossible but you should expect a nice reasonable improvement.

My english is not good and maybe I don't explain to you good about to come back to my pre-op .....

I Know it is almost impossible to return perfectly how I was before, but I would like to Know If I can have a normal chin again with my S form again...the doctors here told me than I can have a normal chin with steroid or with surgery taking out the scar tissue...( the  more prominent part is right under my  bottom lip I suppose it  is because of the scar inside my  mouth) this part is a bump it is soft but it doesn't shink.
I know you are an expert in faces and I know you are a very good doctor for this reason i would like your opinion too.

I don't want live with this for the rest of my life.

About your questions:

The doctor in my country put a implant .But 18 days later he cut the implant and put again (because he told me that it was to big for me) and then 15 days later it was too inflamated and I needed  to come back to Atlanta and I ask him to remove it and he did it.
Then 3 days later I had a lot fluid inside and he take out the liquid with a needle . And was all good but a week later , a hard ball form in the chin.
He told me that the ball would go away  on its own in 3 months. But  the ball improved  a little but not so much....the inflamation is a lot better but now the problem is the scar tissue.

I think that if the part under my lip improves, my chin will look almost normal.

Hi Maria,

Sorry to hear all that you have gone through.  A good plastic surgeon should know ahead of time what size implant will fit best on his or her patient.  It does sound like too much was done to your chin too quickly and that will create lots of scar tissue.  In such cases, you have to be under the care and supervision of a good plastic surgeon to make sure things are not infected and that the area is stable. 

Allowing time to pass and using steroid injections to soften the scar tissue is a good thing usuallly before rushing back into surgery again.  Once time has passed (at least 12 months from the last surgery) and enough steroids have been used, surgical scar removal may be a good option.  Although you will not get back your exact old chin, your surgeons should be able to improve the chin significantly. 

See what Dr. Silver recommends. I wish you luck.
