Radiesse
Question: Hi Dr. Naderi, I am very interested in Artefil. I've had Radiesse several times and it doesn't usually last more than a few months for me (in the NL folds). My question is: a) what is the total cost of the injections (assuming I'm not allergic) and b) if this substance is permanent, won't it look weird as my skin begins to lose its elasticity?

HI, I am surprised Radiesse only lasts a few months for you. It should last at least a year if not more. Injection placement and amount has a lot to do with the longevity of the substance as well. Artefill is $1200 / 0.8cc syringe Most patients need 1, 2 or 3 syringes injected several months apart after the initial skin test. It all depends on the depth of your nasolabial folds. The reason the permanent injection in the nasolabial folds wont look bad as you age is that the area injected does not move. Its a static anatomic area. Also, the substance shoould be "feathered" into the soft tissue and not placed in one large "clump." I hope this makes sense.

Hi Douglas, I hope you are not in the U.S.A., because Aquamid is illegal in the U.S. and it is not an FDA approved substance. Regardless, what is done is done. Aquamid is a permanent particulate filler. As such infection is possible even by itself. People can get infections of their skin with just a plain needle penetration. In such situations, antibiotics often clear up the infection quickly. When there is an implant (and a permanent filler is a kind of implant) then any bacteria can adhere to the material and sometimes its very difficult for antibiotics alone to clear up the infection. So basically, Restylane on top of Aquamid in theory is fine. But it does SLIGHTLY increase the risk of infection. You have to make that final decision. If you would like to send me some pictures through the "contact us" page then I may be able to give you other suggestions - perhaps. I would recommend you see a few other local docs and discuss options. Supposedly Aquamid can be removed with dilution and aspiration although I have never seen that.

Question: Good Afternoon Dr. Nederi, I have received perlane injections on two separate occasions to address grooves across my cheeks. It works for a while but of course I have to keep getting them done and it's unpleasant and expensive. My plastic surgeon recommends a mid-face lift, which I am not at all comfortable with, rather than cheek implants. I don't see the point in a midface lift if I don't have jowls and such that seem to warrant something that major. Plus, I haven't heard about great long term results when dealing with soft tissue lifts like the midface lift. When I look at my face, it appears that I have no cheeks at all. What little I had when I was younger have simply melted away. When, in your opinion, is a midface lift warranted over cheek implants? Thanks,

Hi Michele, To improve the nasolabial folds the easiest and most common approach is to use injections such as Restylane, Perlane, Juvederm, Radiesse, Collagen, etc. Fat injection is also a great option but its a very technique dependent procedure. Good "coleman" technique can give permanent results but poor technique will have very little long term results past 6 months. Artefill is an alternative which is a permanent filler. If the midface atrophy is evident then again fat injections are an option as is malar or submalar implants. These procedures are geared toward enhancing volume. If the midface fat and soft tissue had fallen inferior-medially towards the nasolabial folds creating even deeper nasolabial folds then a midface lift is an option. Many surgeons prefer to do some fat injections along with the midface lift to address the descended tissue and the loss or lack of volume. The results of Midface lifts last about 3-7 years. This procedure will slightly improve jowls as a side effect but it is not geared towards improving the jowls. A cheek lift or facelift is the proper surgery to improve jowls.
