The eyelid lift, technically known as blepharoplasty, is a plastic surgery procedure designed to correct puffy, sagging or tired-looking eyes. The procedure restores a more youthful and refreshed appearance to the eye area by removing excess skin, fat and muscle. The eyelid lift may be performed on the upper or lower eyelids or on both eyelids, and it may be combined with skin resurfacing. Good candidates for the eyelid lift include healthy patients who are looking to improve the appearance of eyelid bags and/or loose eyelid skin

Dr. Kulak occasionally performs the eyelid lift in the office but most often it is done at one of the INOVA system hospitals, which include Fair Oaks Hospital, Loudoun Hospital, Fairfax Hospital, and Mt. Vernon Hospital. The surgery takes about thirty minutes to complete when treating just the upper eyelids, and up to one and a half hours for both the upper and lower eyelids. Anesthetic options include MAC (monitored anesthesia care), general anesthesia, and local anesthesia.

The Procedure

Incisions for the eyelid lift may be made in the creases of the upper eyelids or, for the lower eyelids, just under the lash line or in the crow’s feet area. Once the incisions are made, excess skin, fat, and a small strip of muscle are excised. It is often necessary to be conservative in the removal of fat in order to prevent hollowing. Younger patients, typically those under thirty-five years of age, who are looking to improve bags only may be candidates for transconjunctival blepharoplasty. For this procedure, a small incision is made inside of the lower eyelid, resulting in no visible scarring, and the small pockets of fat are removed. Prominent tear troughs may be addressed by repositioning rather than removing excess fat so that it covers the tear trough area.   Alternatively, fat injection can be done to reduce the hollowness and dark shadows from tear troughs.

After Surgery

Patients may experience a sore or tight feeling in the eyelids after surgery, though any discomfort can be controlled with medication. Swelling and bruising are common and you can help to reduce swelling by keeping the head elevated as much as possible for the first few days. Bruising usually fades within two to three weeks. Dryness, itching, and burning of the eyes, excessive tearing, sensitivity to light, and blurred or double vision may also occur during the first week. The stitches will be removed within seven days. Work can usually be resumed within seven days and more strenuous activities within two weeks.
