Hello Dr Naderi,

I am a 48 year old male - I recently lost about 25 lbs and have very low body fat, but developed loose skin on the neck.

1) Is there a chance that this will correct over time as the weight loss has been recent - I am in excellent heath, eat very healthy and excersize daily.

2) I noticed you have a special "mini neck lift" procedure, would it be practical for me to travel from California to see you or do you recommend colleagues in other parts of the country?

Thank you for your time and response.

Ed

Answer

Hi Ed,

At 48 it would be difficult to imagine the skin tightening on its own after weight loss.  The proper management depends on the degree of laxity and its location.  Neck lift is great when the skin excess is excessive, and Mini Neck lift (Submentoplasty) is great when the skin excess is mild to moderate.  If you email me some pictures from the "contact us" section of this website, I will be glad to give you some suggestions.  I have many patients that come from California, Florida, Midwest, Canada, & Europe.  But, its always easier for patients to try to find a good plastic surgeon close to home.

Dr. Naderi - I am a thin 45 year-old woman with high cheekbones, a nice jawline (no jowls) and until a few months ago, a nice neck and chin (no sign of a double chin).  I now have what is either loose skin or a muscle band going from under the center of my chin down the center of my throat.  My dermatologist thinks that it is a muscle band and gave me a Botox injections a few weeks ago.  While the rest of my neck looks fine, I haven't noticed much improvement in the line/fold down my chin and part of my throat (and there isn't any improvement in the angle where the chin/neck meet).  How can I tell if it is a muscle band or if it is loose skin?  And if it is loose skin, what are the surgical options to eliminate it?  I don't want to have to go through a facelift and I'm not certain that I would be a candidate for one.  I am not certain if I would even be a candidate for a neck lift given that the rest of my neck and my jawline is still nice.  Would laser treatments or Thermage help?  Thank you very much.  Lilly

Answer

It definitely does not sound like you need a neck lift or a facelift.

The way to tell if what you are seeing is muscle or just loose skin is to contract your neck muscles (platysma) and look to see if that band gets more pronounced or tighter.  If the answer is 'yes' then you most likely do have platysmal banding.  Botox should improve it but that is not always the case.

Whether what you see is loose skin or the medial border of the platysma muscle, the surgical treatment in someone like you would be ""Submentoplasty.""  You can read more about this procedure in the website.  It is a 45 minute operation done through a very small ""T"" shaped incision under the chin with wuick recovery and fantastic results in someone who is in good shape otherwise.
