Dear Dr. Naderi,

I have read of continuing swelling in post surgery patients.  I am a certified lymphatic therapist and have provided Manual Lymphatic Drainage as post operative treatment with great results.  Are you familiar with the speedy recovery from this therapy?  Are you including this as a part of your protocol?  May I send you my resume?  I have recently moved to the Fredericksburg area, and am interested in expanding my services in the plastic and cosmetic surgery fields.

Hi,

Thanks for the note. The vast majority of my practice is Rhinoplasty and with the closed scarless approach, as well as precise meticulous surgical technique, swelling is very minimal.  I do not see any major role for your services in my practice.  I would assume the plastic surgeons who heavily concentrate on body procedures would benefit more from your lymphatic drainage and massage technique.

Hi Doctor,

My name is Douglas and I have a question regarding fillers.

About a month ago I did Aquamid fillers on my cheek. However the doctor who did the procedure did not do a good job and I can still see the lines pretty clearly. When I went back to him, his explanation is simply he did not do it well and offer to do it again with another type of filler called Restylane. From what I read, Aquamid is a permanent filler and it should not be used with other type of fillers. The fact that I have only done the Aquamid filler only recently adds on to my concern. However my doctor insist that there are no problems involve.

I would like to know the following:

  • Is what my doctor say true?
  • Is it really ok to use other fillers (Restylane in this case) on those areas done with Aquamid?
  • What is the effect in the long term?
  • Will my face be distorted after a few years because of this mix?
I really hope to have a second opinion from other doctors who are experience in fillers. Hope you can help me with my queries.

Hi Douglas,

I hope you are not in the U.S.A., because Aquamid is illegal in the U.S. and it is not an FDA approved substance.  Regardless, what is done is done.  Aquamid is a permanent particulate filler.  As such infection is possible even by itself.  People can get infections of their skin with just a plain needle penetration.  In such situations, antibiotics often clear up the infection quickly.  When there is an implant (and a permanent filler is a kind of implant) then any bacteria can adhere to the material and sometimes its very difficult for antibiotics alone to clear up the infection.

So basically, Restylane on top of Aquamid in theory is fine.  But it does SLIGHTLY increase the risk of infection.  You have to make that final decision. 

If you would like to send me some pictures through the "contact us" page then I may be able to give you other suggestions - perhaps.  I would recommend you see a few other local docs and discuss options. 

Supposedly Aquamid can be removed with dilution and aspiration although I have never seen that.

Hello,

    I am currently looking for a doctor experienced in Otoplasty. I am looking for experience and quality.  I would be appreciative if you could answer the following questions:

How many otoplasty procedures have you performed?  Is your incisionless technique proven as effective as a conventional technique?

I saw the cost as $2800.00 excluding hospital, anesthesia, etc type fees.  What is a ballpark, all inclusive cost for otoplasty?

Hi,

I only perform facial plastic surgery and I have done almost a thousand ear cases.  The incionless otoplasty technique is a very good tehcnique IN THE RIGHT PATIENT.  Not all patients are candidates for this.  It is good for creation of the antihelical folds but not good in cases where the conchal bowl needs to be set back.  I would have to see you or at least some pictures to be able to tell you more information including cost.  There is not one set fee with any of my procedures as no two patients are identical.  You have to fit the surgery to the patient and not fit the patient to the surgery.  The average cost though is around $5-6K for my otoplasties although there are many less expensive surgeons out there. 

I would be glad to see your pictures or examine you and go over your best options.

Hi Dr. Naderi,

I have keloid scars which are very old, most are flat with one or two still raised. The flat ones are still reddish in color and I was wondering if you have any effective treatments for that. I have gone through almost 2 years of pulsed dye laser treatments in DC with no result. I went to London for 1 year where the doctor gave me monthly shots of higher dosed kenalog along with erbium yag laser which yielded greater results. Do you offer that in your office and if so, what is the price range? How much is the consultation fee?

Hi,

I would like to see some pictures of your scars if you can email them to me or you can come in for a consult.  Keloid and Hypertrophic scars resond the best initially and then the response can plateau.  Steroid injections work well for raised scars and PDL should work as well depending on the nature of the scar.  CO2, Erbium lasers are options as in a Phenol peel but often Dermabrasion is the best choice.  Some scars need to be excised and resutured.  It just all depends on the nature of your scar.
