Following the advice of my plastic surgeon I received fat transplant to the upper eyelids in the form of injections. The results were disastrous; the right eye especially has calcified lines of fat or scar tissue and the inner upper corner an especially noticeable nodule. My question is have you ever successfully removed injected fat from the upper eyelids? If not, is there someone in your circle of professionals experienced with this type of problem? Thank you in advance.

Hi,

I have seen plenty of poorly done fat injection and around the eyes and lips, nodules can become a large problem.  Other areas are more forgiving. 

I would have to see you to determine what is the best approach.  Most of the time the area can be improved greatly with direct excision of the nodules or hypertorphic deep scars.

Hi Dr. Naderi,

I am considering a lower lid blepharoplasty to remove under eye bags.  I am 44 years old.  Do I have to worry about my eyes looking hollow after the procedure?  Also, what type of anesthesia is used and is the procedure performed in your office or a hospital?  Approximately how many lower lid bleph procedures have you performed?  Thanks so much for your response!

Hi,

As with any cosmetic surgery, the lower lid blepharoplasty can be done properly or poorly!  If done poorly or over aggressively then lower lid hollowing can be one of the complications. 

The key in eyelid surgery is not to remove too much fat or every single extra pinch of skin.  If too much fat is removed then the eyes may become hollow and dark and sunken in.  If too much skin is removed then lid retraction or ectropion or inability to close the eyes or dry eyes can occur. 

The surgery for the lower lids can be done "external" or "transconjunctival."  I typically perform these surgeries at the Hospital but occasionally I may make exceptions and perform them in the office - occasionally!

Anesthesia for such cases are usually local or twilight but general is an option. I have probably performed hundreds of eyelid surgeries over the last 9 years but I do not know the exact number.  I hope this helped clarify some of your questions.

I am 23 years old and I have puffy upper eyelids.

I am 23 years old and I have puffy upper eyelids. Am I too young for eyelid lift surgery? Could I use brow lift instead, so I’d get both a better shape eyebrow and eyelids?


Answer

You are very young to need an upper lid blepharoplasty.  The exception would be an asian patient who wants to "westernize" her eyelids or a young patient with LOWER eyelid bags which are hereditary.  A brow lift may be a possibility but there again you are so very young that it would be very unusual for you to NEED a browlift.  There are some medical conditions of the eyelids that can cause "puffiness" and I would recommend you see an Opthalmologist to make sure this is not the problem.  Be careful and be conservative.  You are very young and you should not do something that you may regret down the line.
